Reporting directly to the Chief Operating Officer and forming part of the Senior Clinical Team, you will provide values-led, strategic leadership across clinical services, quality governance, and education. St Roccos ethos is to support people in the Warrington community from the moment they are diagnosed with an illness that is no longer curable. We look after people over the age of 18 years with any kind of life-limiting illness, this can include heart failure, respiratory conditions, and neurological diseases.

Outreach Matron

Job Summary

We are seeking an experienced and dynamic senior nurse to join St Rocco's Hospice as Outreach Matron. Reporting to the Chief Operating Officer, you will provide visible clinical and professional leadership across our Outreach Services, including the Palliative Virtual Ward and Integrated Palliative Care Hub.

As an expert practitioner and senior clinical leader, you will be responsible for delivering high-quality, evidence-based specialist palliative care, supporting patients from assessment through to treatment, care planning and evaluation. You will demonstrate advanced clinical decision-making, sound judgement and a commitment to continuous service improvement.

Working closely with the In-Patient Unit (IPU) Matron, Medical Director and senior clinical teams, you will lead the development of clinical practice, support operational delivery, promote innovation and ensure services meet the needs of patients, families and carers across Warrington.

The successful candidate will be a highly visible role model who champions hospice values, promotes a culture of safety, quality and learning, and supports staff to deliver outstanding patient-centred care.

Main Duties of the JobProvide strategic and operational leadership for Outreach Services, including the Palliative Virtual Ward and Integrated Palliative Care Hub.Work collaboratively with the Chief Operating Officer, Medical Director and senior clinical team to support delivery of the hospice's clinical strategy and operational objectives.Ensure high standards of care are maintained in line with NMC requirements, CQC standards, hospice policies and best practice guidance.Lead and support clinical teams to deliver safe, effective and compassionate specialist palliative and end of life care.Exercise advanced clinical judgement and decision-making in the assessment, planning, implementation and evaluation of patient care.Act as a visible clinical leader and role model, promoting a positive culture of learning, innovation, quality improvement and professional accountability.Support and develop staff through supervision, mentorship, appraisal, education and performance management.Manage and support outreach clinical teams, ensuring appropriate staffing levels, skill mix and effective workforce utilisation.Work alongside the IPU Matron to ensure seamless service delivery and appropriate clinical support across hospice services.Maintain oversight of clinical governance, risk management, incident reporting and quality assurance processes.Promote evidence-based practice through audit, service evaluation and continuous improvement initiatives.Ensure robust systems are in place to monitor quality, compliance and patient experience outcomes.Provide specialist advice, guidance and support to healthcare professionals across hospice, community and partner organisations.Manage a clinical caseload when service demand requires and support complex patient assessments and care reviews.Maintain accurate clinical documentation in accordance with professional, organisational and legal requirements.Work collaboratively with primary care, acute trusts, social care and voluntary sector partners to ensure effective integrated care pathways.Support patient and family engagement, ensuring services are responsive to feedback and local population needs.Contribute to workforce planning, service development and delivery within agreed financial and resource frameworks.Ensure compliance with safeguarding, infection prevention, health and safety, information governance and data protection requirements.Produce and present activity, quality and performance reports, highlighting risks, trends and service developments.Promote the hospice, its services and charitable objectives, acting as an ambassador for St Rocco's Hospice.Person RequirementsThe post holder will be an experienced senior registered nurse with significant leadership experience within palliative care, community care or a related clinical setting. They will demonstrate excellent communication skills, strong operational management capability and a passion for delivering exceptional patient-centred care.

The ability to build effective relationships, lead teams through change and drive service improvement is essential, together with a commitment to the values and mission of St Rocco's Hospice.

Qualifications Essential

Education/Qualifications 1st level Registered General Nurse. First Degree in Nursing or willing to work towards. Relevant additional qualification/s in palliative care. Senior Healthcare Leadership Significant experience of working in a community environment Demonstrate a high level of experience in delivering palliative care Significant experience of managing and developing people Experience of leading and managing change Experience of mentoring staff Experience of using IT software Independent working Organisational and interpersonal skills Record keeping Time management Communication Clinical Audit Report writing and presentation skills Critical thinking Advanced Communication skills training Ability to work flexibly to demands and managing an unpredictable work pattern.
