Job summary
To provide continuing responsibility and accountability in the delivery of a dedicated Liaison Psychiatry Service and working closely with the Sheffield teaching Hospitals, mental health services and other stakeholders.
The Mental Health Practitioner (Registered Mental Health Nurse, Occupational Therapist or Social Worker) will carry continuing responsibility for the assessment of mental health needs and the development, implementation and evaluation programmes of mental health nursing care in the management of a defined caseload. This will include liaison with other agencies and supervision also teaching of other staff and students.
Main duties of the job
The Liaison Psychiatry Service is a PLAN affiliated, multi-disciplinary and innovative team. You will be expected to work towards accreditation as part of your ongoing practice.
The Mental Health Practitioner will need to conduct robust triage and comprehensive biopsychosocial assessments of need for the individuals who present with a wide range of needs, most often in mental health crisis.
The Mental Health Practitioner will have knowledge of working with individuals who experience suicide, self-harm, acute mental illness presentations and dementia / confusion /delirium.

Job description Job responsibilities
Able to work independently in a number of environments as an autonomous practitioner.
Provide comprehensive specialist mental health assessment and risk assessment of a wide range of service users. Identification of referred patients' mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patients over a 24-hour period. Regularly review caseload management with their clinical supervisor
Maintain accurate records about the care and treatment offered.
Review and evaluate professional standards of profession in conjunction with the Senior Leadership Team.
Ensure that the developing requirements of the Mental Health Practitioner profession are understood by the wider team.
Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Trust policies.
Adhere to confidentiality principles and maintain the rights of individual patients.
Contribute personal and professional skills, therapeutic use of self and those of other staff under their supervision, to ensure the development of the Mental Health Team
Identify and manage risks in variety of settings.
Offering support and advice to families and carers and acting as an advocate for clients, where appropriate.
Manage a caseload by the effective use of time and resources.
Provide timely triage of service users in A&E and across the Teaching Hospitals.
Provide constructive supervision in clinical managerial and educational needs in relation to staff development and risk Management.
Influence/make and implement changes to clinical practice, locally and trust wide.
Participate in individual appraisal performance reviews and the construction of personal developments plan.
Person Specification
Knowledge Essential
To possess excellent risk assessment and management skills. Demonstrable oral and written communication skills. Ability to assess plan, implement and evaluate nursing care. Effective time management skills. Skills in assessment, formulation, and treatment planning for individuals in crisis. The ability to work and make decisions autonomously. Knowledge and understanding of MHA and other relevant legislation such as MCA. Knowledge of relevant treatment modalities Awareness of other agencies within wider community Skills in team working and shift co-ordination. Working knowledge of in-service Governance.
Qualifications Essential
Registered Mental Health Nurse with current registration with NMC /Social Work Qualification/Occupational Therapy Qualification. Evidence of continuous professional development since qualification. Evidence of higher practice skills in mental health care across the age range.
Experience Essential
Experience of working in acute mental health care settings. Demonstrable experience of collaborative working within a multi-professional environment. Active involvement in promoting Clinical Supervision. Track record in engaging with service users and carers. Experience/knowledge of implementation of the Mental Health Act.
