Job summary
An exciting opportunity has arisen for a motivated and enthusiastic Band 5 Physiotherapist to join the Brent Home First Service.
The Home First Service supports timely and safe discharge from acute and urgent care settings, helping patients to recover and remain independent within their own homes. Working as part of a multidisciplinary team, you will provide assessment, treatment, and rehabilitation for patients with a range of complex health and social care needs.
You will manage your own caseload, contribute to clinical decision making, and work closely with acute trusts, primary care, community services, and social care teams to deliver integrated, person centred care and achieve positive outcomes for patients.
This role offers an excellent opportunity to develop your community rehabilitation skills within a supportive team environment, with access to regular supervision, training, and professional development opportunities.
The service operates 7 days a week between 8:00am and 8:00pm, including weekends and bank holidays. Full time staff are expected to participate in a rota covering early, late, and long day shifts as required.

Main duties of the job
o Provide physiotherapy assessment, treatment, and rehabilitation to patients within the Home First Service, supporting safe discharge and recovery at home.
o Manage a defined caseload under appropriate supervision, delivering evidence based interventions to meet individual patient needs.
o Support patients to maximise their mobility, function, independence, and overall wellbeing within the community setting.
o Work collaboratively within a multidisciplinary team to contribute to assessment, care planning, and coordinated service delivery.
o Identify, monitor, and appropriately escalate clinical concerns, risks, and changes in patient condition to senior colleagues.
o Maintain accurate, timely, and comprehensive clinical records in accordance with HCPC, CSP, and organisational standards.
o Participate in clinical supervision, training, and continuing professional development activities to maintain and develop professional competence.
o Contribute to the delivery of a high quality, responsive, and patient centred rehabilitation service across Brent.

Education/Qualifications Essential
Diploma/Degree in Physiotherapy Current AHP with UK HCPC registration
Desirable
Registration with the Health and Care Professions Council
Experience Essential
Relevant work experience in a variety of clinical placements. Evidence of up-to-date CPD oExperience of working as part of an MDT Use of functional assessment and activity analysis.

Evidence of previous employment as an Occupational therapist, apprentice, assistant Experience of delegation to support staff
Skills and Knowledge Essential
Knowledge of current best practice in PT, demonstrate clinical reasoning, applying theory to practice. Ability to work single-handedly with individuals and groups.
