Help connect people to the care they need and make a measurable impact on community health. Full-time, benefit eligible position. Monday - Friday 8:00am-4:30pm. *Remote position open to Missouri residents only. The Population Health Specialist plays an important role in helping patients connect with the right healthcare services at the right time. Through proactive outreach, care gap identification, follow-up, education, and collaboration with the healthcare team, this role helps strengthen patient engagement and support improved health outcomes. The Specialist also uses patient and program data to monitor progress, support quality initiatives, and contribute to value-based care efforts. This is a great opportunity for someone who enjoys building relationships, using data to identify opportunities for better care, and helping people navigate available health services. You'll have the opportunity to make a meaningful difference by helping patients stay connected to preventive care, chronic disease support, screenings, wellness visits, and other services that promote their overall well-being. Conduct proactive outreach to patients and individuals who may benefit from Health Center services. Help patients understand and access available healthcare services and resources. Identify and address care gaps through outreach, education, scheduling, and follow-up. Follow up with patients who miss preventive or other important appointments and assist with rescheduling. Use patient and program data to track outcomes, identify trends, and support quality improvement initiatives. Support clinical quality measures and value-based care initiatives through patient engagement and follow-up. Build trusting relationships with patients and caregivers that encourage continued engagement with care. Collaborate with healthcare teams to support coordinated, patient-centered care. Contribute to program initiatives, performance monitoring, and reporting. Maintain accurate documentation and comply with applicable federal and state privacy requirements. Apply approved crisis intervention techniques, as needed, in accordance with current certification and organizational protocols. Perform other duties as assigned by the Director of Population Health or senior management.
