Main duties of the job

We have an exciting opportunity for a Nursing Associate to join our Primary Care Network team. The Nursing Associate will work under the supervision of the Practice Nursing Team within the allocated PCN. Leadership and support will be provided by the PCN Manager, Clinical Director, and member practices. The Nursing Associate will work within the community to support the everyday running of the PCN practices. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience. The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.

Job description Job responsibilities

Work within the PCN to provide and monitor care, alongside the wider PCN team, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervisionImprove safety and quality of care at every opportunityUndertake additional clinical training as required by the PCN to support the Network Contract/DES Services SpecificationSupport registered nurses and the wider team to enable them to focus on the more complex clinical careTo support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experienceProvide support to the PCN with regards to their Network Contract DES Services Specification including CVD prevention and tackling health inequalitiesDevelop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health servicesProvide support and supervision to trainee nursing associates, healthcare assistants, apprentices, and those on learning placements appropriate to level of competenceAccurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse ratesEngage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationshipsCommunicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as requiredCommunicate complex and sensitive information verbally and in writing. Adapt and tailor communication skills to the need of the listenerTo act as patient advocate during practice or PCN meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriatelyDevelop an understanding of MDT structures and members rolesSupport patients to improve and maintain their physical, mental, and behavioural health and well-beingProvide advice to patients including promoting self -management where appropriate and signposting patients to other PCN colleagues, local community, and voluntary sector servicesMaintain accurate and contemporaneous patient health recordsSkills and training needs to be updated through continuous professional development, attending regular educational updates, via e-learning and face to face sessions as appropriateWork in adherence to NMC Code for Nursing Associates, within own level of competence and the protocols and policies of the practices/PCNMaintain a good working knowledge of Health and Safety procedures, fire safety and mandatory training as per practice and PCN policies. Participate in policy development and data collection where appropriateWork flexibly to meet the needs of the service and patients. Adapt to what may be potentially challenging and changing situations at timesWilling to undertake other duties deemed appropriate to a Nursing Associate following correct training/guidance and agreed by the post holder and their line manager

Qualifications Essential

Microsoft Office Qualification or extensive Microsoft Word & Excel experience Good standard of education Specific qualification and training requirements as specified in the Nursing and Midwifery Council Standards of Proficiency by having undertaken and completed the two year foundation degree, recognised by the Nursing and Midwifery Council. Registered with the NMC and revalidation is undertaken in line with NMC requirements.

Experience Essential

Understanding on human needs physical, emotional & social Proven ability to recognise and manage risk Knowledge of the needs of vulnerable adults, safeguarding and the associated legislative framework Understanding of basic health and social care terminology Good interpersonal skills Ability to communicate confidentially with staff of all seniority levels Ability to work well across teams Good time management and prioritisation skills, ability to work to strict deadlines. Ability to work on own initiative without direct supervision, understanding where clinical input is needed, recognising own limitations. IT skills and experience using Microsoft packages Coordination of services from a variety of organisations. Communication and relationship building Experience in Health and or Social care Experience of developing relationships with a wide variety of people Highly organised.

Desirable

Familiarity with IT systems currently in use in health and social care settings. Experience of health and social care assessments Demonstrated successful outcomes delivery within determined timeframes
