PURPOSE:
This position provides multi-functional duties related to service, assistance, support, and supervision to one or more clients in a residential care setting. Successful candidates will work in client homes or in one of our care homes
NOTE: This position requires an active PCA/CNA/HHA license in GA.
ESSENTIAL FUNCTIONS:
Follows the patient's individual service plan in providing one-on-one support and companionship, which may include (but not limited to):Daily wellness checks to monitor and report any change in condition.Housekeeping to include daily straightening of the patient's residence, with weekly light house cleaning.Incontinence management includes assistance to the bathroom, providing assistance, and hygiene as needed.Assistance with activities of daily living, including bathing, dressing, eating, toileting, hygiene, and mobility.Assists in escorting patients to meals, activities, events, or nature walks.Assists in updating each client's individual service plan and charting.Reports concerns about clients and residents to the Nurse-on-Duty or the Director of Care Services.Encourages socialization and participation in the community.Assist residents with participation in activitiesMay accompany clients related to activities outside of the client's homePromote the safety and well-being of patientsAssists patients to and with meal service in the dining room as needed.Requirements
REQUIREMENTS:
High School diploma or equivalentCertifications: Active PCA/CNA/HHA certification in the state of Georgia (Must have at least one)Proficient in verbal and written EnglishExperience as a primary caregiverAbility to work schedules assigned in a 24-hour operation, including weekendsGood communication and telephone skillsMust successfully complete:
TB skin test or chest x-rayState fingerprint and FBI background checkBasic First Aid trainingPREFERENCES:
One plus year of direct care experiencePC skillsPrevious customer service experienceObservation and reporting skills1 + years of long-term care experienceActive Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in the state of GeorgiaActive Driver's License and ability to pass DMV background checkDependable transportation Benefits
