Job SummaryWe are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including pediatrics, seniors, and individuals with special needs such as dementia or hospice care. The ideal candidate will be committed to delivering personalized care while adhering to all safety and regulatory standards.ResponsibilitiesAdminister medications and treatments accurately according to physician ordersMonitor patient vital signs and observe changes in conditionAssist with patient assessments and document findings in electronic health records such as Epic or Cerner systemsProvide wound care, phlebotomy, and other clinical procedures as neededCollaborate with interdisciplinary teams to develop and implement patient care plansEducate patients and families on health management and post-discharge care instructionsMaintain a safe, clean, and organized environment in compliance with healthcare regulationsSupport patients with activities of daily living in settings such as assisted living or hospice careWork with children, seniors, or patients with complex conditions like dementia or pain management needsSkillsProven experience in nursing home, hospital, or acute care settingsStrong knowledge of medical terminology and medication administration protocolsExperience with electronic health records systems such as Epic or Cerner is preferredFamiliarity with specialized care areas including dementia, pediatrics, hospice, or senior careExcellent patient care skills with a compassionate approachAbility to perform laboratory procedures such as phlebotomy when requiredEffective communication skills for working with patients of all ages and collaborating within healthcare teamsKnowledge of case management principles and pain management techniques is advantageousAbility to adapt quickly to different clinical environments and handle multiple priorities efficiently
