Job summary
The Proactive Neighbourhood Care and Remote Monitoring service will support people living with frailty, complex needs and those who are housebound. The service will combine proactive home visiting, care coordination, remote monitoring and links into wider neighbourhood and Local Authority support.
The Service Manager will lead the mobilisation and day-to-day delivery of the integrated proactive neighbourhood and remote monitoring service. The postholder will manage the multidisciplinary workforce, ensure safe and effective pathways are in place, deliver agreed activity and outcomes and maintain strong relationships with practices, Local Authorities and NHS partners.
Main duties of the job
Lead mobilisation from recruitment and induction through to go-live and business as usual. Manage the care coordinator, HCA, bank clinical workforce and driver bank, with clear supervision and accountability. Coordinate the shared workforce across proactive home visiting and remote monitoring to avoid duplication and improve value for money. Ensure referrals, triage, visits, monitoring, escalation and discharge processes are safe, clear and consistently followed. Work with Clinical Leads, GPs, paramedics, Local Authority teams, practices and partner organisations to resolve issues quickly. Monitor activity, utilisation, patient outcomes, workforce capacity, quality, incidents, complaints and budget performance. Produce accurate reports and dashboards and take action where delivery is off plan. Ensure SOPs, risk assessments, information governance, safeguarding and CQC requirements are embedded.
Support continuous improvement and the ongoing development of the neighbourhood model.

Person Specification
Qualifications Essential
Degree level qualification or equivalent relevant operational management experience. GCSE English and Maths, grade C/4 or above, or equivalent.
Desirable
Leadership, management, project management or quality improvement qualification.
Experience Essential
Significant experience in healthcare service/operational management Experience managing staff and multi-site services Strong knowledge of primary care systems (EMIS or similar) Experience managing rotas, staffing, and capacity Strong organisational and problem-solving skills Ability to work under pressure and manage competing priorities Excellent communication and stakeholder engagement skills
Desirable
Experience in primary care, urgent care, or PCN environments Experience working with NHS systems and partners (ICBs, Trusts) Experience with service mobilisation or pathway development Knowledge of quality improvement, audits, and governance processes
