About this role

Job summary

The Proactive Neighbourhood Care and Remote Monitoring service will support people living with frailty, complex needs and those who are housebound. The service will combine proactive home visiting, care coordination, remote monitoring and links into wider neighbourhood and Local Authority support.

The Healthcare Assistant will provide practical clinical and patient support across proactive home visiting and remote monitoring. The role will include onboarding calls, routine wellbeing contact, basic observations, equipment support and home visits completed within agreed protocols and under appropriate clinical supervision.

Main duties of the job

Make onboarding and routine follow-up calls and explain how the service and monitoring equipment work. Undertake agreed observations such as blood pressure, pulse, oxygen saturation, temperature, weight and other basic checks within competence. Visit patients at home to support assessments, equipment setup, monitoring and agreed follow-up tasks. Record all information accurately and escalate abnormal readings or concerns immediately using agreed pathways. Support patient activation, self-monitoring and confidence in managing long-term conditions. Work closely with the care coordinator, GPs, frailty prescribers and wider neighbourhood team. Maintain equipment, supplies and infection prevention standards. Identify and escalate safeguarding, deterioration or welfare concerns. Contribute to service data, audits and patient feedback.

Person Specification

Experience Essential

Experience as an HCA, clinical support worker or similar patient-facing role. Competent in basic clinical observations or willing to complete training before independent practice. Good communication skills and confidence supporting older or vulnerable patients. Ability to follow protocols, recognise limits of competence and escalate concerns. Full UK driving licence and access to a suitable vehicle for work.

Desirable

Experience in primary care, community care, frailty, home visiting or remote monitoring. Phlebotomy, ECG or additional clinical skills.
