Job summary
Provision of specialist, high quality, expert Multiple Sclerosis care. Developing appropriate guidelines and clinical pathways for patients under the care of the multiple Sclerosis service.
Liaison with the multidisciplinary team(s) and other health agencies.
Provide care across the whole disease trajectory including assessment of newly diagnosed and those with an established diagnosis including treatments, onward referral/discharge and end of life care. Supporting ward staff to safely discharge patients back to their homes.
Carry out telephone assessments, face to face assessments of patients with MS and formulate plans of care and support.
Provide specialist information to patients, carers and other healthcare professionals on the management of Multiple Sclerosis.
On completion of training, to safely carry out Non Medical Prescribing Practice to support the Consultants in managing patients with MS.
***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***
Main duties of the job
Assess, plan, implement and evaluate expert nursing care and provide support to people living with Multiple Sclerosis and their carers, having continuous responsibility for co-ordination and organisation of individual treatment programmes throughout the illness trajectory
Develops and manages a caseload of patients with complex needs. Competent to undertake advanced nursing assessments of the patient's needs
Act as an Independent Nurse Prescriber, able to distribute repeat DMT prescriptions for MS Patients with support from the MS Consultant team (not initiating new treatments) and monitoring the safety of the treatment regimes.
Competent with the checking and administration of Iv Infusions to MS patients if required.
Act as an expert source of knowledge and information for people with Multiple Sclerosis.
The post holder liaises effectively with all members of the Multidisciplinary team within the acute hospital and primary care settings and supports developmental roles that enhance team capacity and resilience.
Provide specialist nursing advice, psychological support and act as a consultancy role to people living with multiple sclerosis, their carers and other health and social care professionals;
The post holder will work autonomously and as part of a team.
Develops the service according to need, undertakes audit and research projects and develops policies and procedures.

Clinical Practice
Provide enhanced clinical care in line with professional registration, code of conduct and defined scope of practice, being responsible and accountable for own decisions, actions and omissions. Plan, prioritise and deliver enhanced clinical care within a defined resource and scope of practice. Undertake holistic patient-centred assessments using available tools, technologies and techniques. Analyse the data arising from the assessment process to inform clinical decision-making. Apply enhanced skills in assessing, planning, implementing and evaluating individual treatment interventions for patients in partnership with members of the MDT. Identify, request and interpret specialist diagnostics within own scope of practice to inform the delivery and management of specialist care for patients and families. Appraise the relevance of available tools and techniques to the clinical situation and own scope of practice. Work within legislation, clinical frameworks and contemptuous evidence-based practice guidelines, outcomes from clinical audit and algorithms to support decision making, Develop, implement and evaluate an enhanced care management plan which may include interventions and referral to other members of the multidisciplinary team or other agencies.

Qualification Essential

Experience & Knowledge Essential
