Job summary
Moorside Surgery is a friendly and successful training practice based in Bradford, West Yorkshire. We have a fantastic opportunity for a Clinical Coder/Summariser to join our multi-disciplinary team. The postholder will be working alongside our current document manager/scanner coder.
We are CQC rated good (June 2026) and rated outstanding for providing effective services and for the care of people with long term conditions - our teams are committed to delivering a high-quality provision to over 8500 patients with a focus on continuity of care.
The Surgery is a satellite Hub Practice offering patients increased access to GP, Nurse, and Physiotherapy appointments on Wednesday evenings. (Evening work is not required for this role).
Main duties of the job
The main responsibilities of the post-holder will be reviewing clinical documents, Lloyd George notes and other letters to identify relevant clinical findings and read coding these documents as appropriate, to ensure accuracy in medical records.

Job Responsibilities Main duties/Key Tasks:
Extract relevant clinical information for coding.
Enter data accurately into the clinical system.
Apply appropriate Read/SNOMED codes.
Ensure confidentiality and data protection compliance.
Liaise with clinical staff for clarification when needed.
Maintain up to date, accurate patient records.
Review and summarise incoming patient medical records.
Support QOF, audits and clinical reporting.
Work to agreed targets and deadlines.
Contribute to efficient record record-keeping and patient care.
Any other administration duties.
Person Specification
Qualifications Essential
GCSEs or equivalent in English and Maths at Grade 4 / C or above Understanding of the NHS in particular primary care Ability to organise workload and proficient in using computers.
Desirable
NVQ Level 2 in Business Administration, Health and Social Care, Pharmacy services, or equivalent relevant qualification Experience of using SystmOne
Experience Essential
An experienced administrator, with excellent IT, time management and organisational skills with a knowledge of medical terminology An experienced clinical coder / note summariser. Ability to communicate effectively, working in a confidential and discreet manner. Ability to multi-task, work effectively and maintain a high level of attention to detail, when under pressure Ability to work as part of a multi-disciplinary team, as well as independently.
