About this role

Job summary

The band 6 Children's Community Nursing Team sister provides care in the home/ community setting that would have been delivered previously in hospital. The post holder will carry their own caseload and provide supervision to other team members, when required. The team is further developing and now provides care to children in school requiring Total Patient as well as supporting existing service to meet the needs of children with chronic and other medical conditions at home thereby shortening or preventing hospital admission and improving school attendance.

Main duties of the job

The role is essential to provide nursing care within the community, being the lead professional for the CYP and linking in with the larger Multi-Disciplinary team including tertiary centres, social care, education and local consultants at the James Paget University Hospital.

The post holder will need to have a valid driving licence.

To develop good communication links with all paediatric wards and specialist liaison nurses to facilitate discharge planning arrangements for continuing patient care and to participate in ward meetings and case conferences as required.

Person Specification

Education and Qualifications Essential

RSCN/RN Child or equivalent Evidence of continued professional development Preparation for mentorship (ENB 988 or equivalent)

Desirable

Management/Team leading qualification

Knowledge and Experience Essential

Experience of shift coordination in an acute setting in the absence of a sister/charge nurse within the last 3 years Paediatric nursing experience in both acute and community settings Acute paediatric nursing experience of working in a team and independently Can show experience of team working Completed extended nursing role

Skills and Knowledge Essential

Ability to work confidently and autonomously and remain calm under stress IT skills Flexible approach to working pattern Excellent communication skills Ability to work within a multidisciplinary team
