RN Nurse Manager-Transitional Care & Rehabilitation Unit

The Highlands Living Center is seeking an experienced Registered Nurse (RN) Unit Manager to lead our fast-paced Transitional Care & Rehabilitation Unit. Our Transitional Care Unit serves residents with complex medical needs requiring short-term rehabilitation, skilled nursing care, and interdisciplinary collaboration. What You’ll Do: · Provide day-to-day clinical leadership for the Transitional Care/Rehabilitation Unit. · Oversee resident care, ensuring quality outcomes and regulatory compliance. · Collaborate with physicians, therapy, pharmacy, and interdisciplinary team members to coordinate care. · Mentor, coach, and support nursing staff while promoting accountability and professional growth. · Monitor admissions, discharges, care planning, and clinical documentation. · Identify changes in resident conditions and implement appropriate interventions. · Promote a culture focused on rehabilitation, recovery, patient satisfaction, and excellence in care. What We’re Looking For: This position is intended for experienced clinical nurses. Required Qualifications · Current New York State RN license. · Previous experience in skilled nursing, rehabilitation, transitional care, sub-acute care, or acute care. · Strong assessment, critical thinking, and clinical decision-making skills. · Proven leadership ability with excellent communication and organizational skills. Preferred Qualifications · Previous Unit Manager, Charge Nurse, or Nurse Supervisor experience. · Experience caring for medically complex rehabilitation or post-acute patients. · Knowledge of Medicare, MDS, quality measures, and long-term care regulations. Please Note: This role is not an entry-level nursing leadership position. Candidates should have significant rehabilitation, transitional care, or strong clinical nursing experience. Nurses without recent experience managing complex clinical patients in a rehabilitation, skilled nursing, hospital, or similar high-acuity setting are encouraged to explore other opportunities within our organization that better match their experience. Qualifications: Education & Experience: Graduate of approved School of Nursing. AAS Nursing degree, required and BSN preferred. NYS RN License. MDS and PRI proficient and 2 years supervisory experience required. Pay Range: $85,000-$110,000 annually
