Job summary

Organisations across the Nottinghamshire Integrated Care System (ICS) have come together to create an exciting new rotation post across health and social care, offering rotations within physical health, mental health and social care in acute, community and rehabilitation settings.

Each rotation cycle will be 6 months, allowing you to learn a variety of skills in a number of different settings. Many of the skills you will learn within this rotation will be transferable and will give you a breadth of experience to help you develop into an experienced therapist and beyond!

Each organisation is committed to your development and growth within Occupational Therapy. You will be supported throughout the rotation with a comprehensive support package including supervision, preceptorship, peer support groups, skills to build, in-service training and more.

Main duties of the job

To work as an autonomous practitioner and undertake comprehensive assessments to identify the appropriate service for the clients referred. To carry out profession specific, and holistic, assessments and evaluation as required. To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs in a community bed. To be responsible for continuing the ongoing development of high standards of clinical practice with evidence-based focus. Able to fulfil physical demands of role, including manual handling applications. To participate in rehabilitation treatment programmes with the MDT To carry out training of other members of staff. Support senior colleagues with the facilitation of student placements.

Job description Job responsibilities

Job Purpose

To contribute to a high quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of adult clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists. To work within Community Beds Team withing Health Reablement service . This may extend or change to other community services in-line with organisational development To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies. To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training. To be responsible for planning a safe discharge from the service. To act as Key Worker as required. The post holder will be substantively employed by Nottingham CityCare Partnership CIC within the Community Beds Team. However this role forms part of an Integrated Care System (ICS) rotational programme and the post holder will be expected to undertake rotational placements across partner within ICS health and social care organisations.

Dimensions

Responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. Working in conjunction with and under supervision of a senior OT and other senior clinical professionals. Lead group sessions where appropriate. Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for client visits, meetings and training. To participate in rehabilitation treatment programmes with the MDT To carry out training of other members of staff. Support senior colleagues with the facilitation of student placements.

Person Specification

Experience Essential

Relevant undergraduate/placement experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal Understanding of multi-disciplinary and/or multi-agency working in a community setting

Community experience/Interest in working in community

Qualifications Essential

BSc/Diploma in Occupational Therapy

Relevant postgraduate education and training Post graduate training / courses

Additional Criteria Essential

This post may require shift working between 8am-4pm, 12-8pm 5 days over 7 including weekend and bank holidays Full driving licence, use of vehicle for work with business insurance

Skills and Attributes Essential

Understanding the concepts of assessment, completing standard assessment of need, able to formulate a care planning and taking appropriate action. Awareness of a range of activity of daily living equipment and adaptations. Effective communication skills (Verbal and written) and good interpersonal skills and IT skills. Ability to involve individuals in a person-centred approach to care. Ability to participate as an MDT team member IT literacy

An understanding of mental health policy and legislation, e.g., Mental Capacity Act, Dementia Strategy Able to supervise and delegate to support staff
