About this role

Job summary

We are looking for a forward thinking Speech and Language Therapist with community experience to join our Community teams based at Hollyfield House, Surbiton. We are an extremely friendly and supportive innovative service. We work on patents own goals and keep patients at the centre of everything we do.

The post is across 2 of our community teams and is to cover maternity leave for a 9 month period, with the possibility of review at the end. Our services at Your Healthcare work closely together, and with other partners, in the provision of a heath and social care service to patients living within the Royal Borough of Kingston.

The Community Neuro Rehab team are a highly respected Multi Disciplinary Team providing specialist neuro rehab to patients in their homes and in out-patient settings. You would be an integral part of this team working to provide a dysphagia and communication element to an efficient and effective rehab service.

The Adult Community Speech and Language Therapy Team provide a mainly dysphagia service to patients from 18 years upwards in conjunction with our broader services in Your Healthcare to ensure patients are supported to stay at home or are supported when they leave hospital.

Main duties of the job

This is a 9 month fixed term role to cover maternity leave, with the option to review at the end of the 9 month period. It is for22.5 hours per week.The usual working hours are 08.00 16.00 and will be across 3 days in the 5 day working week.The plan is for the postholder to work 2 days in our Adult Community Speech and Language Therapy Team and 1 day in our Community Neuro Rehab Team (CNRT).Details of the working week can be discussed at interview.

You will be based at Hollyfield House, Surbiton although you will also need to be able to travel to our other sites and places of residence as needed.

Main duties include:

To provide a Speech and Language Therapy Service consisting of specialist assessment, diagnosis, direct and indirect interventions and advice to a wide range of patients and their carers, including those with complex difficulties including those of a neurological origin, who have communication and swallowing needs. To formulate goal centred treatment plans and delegate appropriately to assistants or generic care assistants who work as members of the multidisciplinary team. To work autonomously and as a member of a multi-disciplinary team, ensuring the communication of information to all health and social care agencies involved, to meet the needs of the patient and plan effective care. To provide supervision and support to junior staff, assistants and students.

Skill, Abilities and Knowledge Essential

Communication and eating/drinking disorders to a high level of competence. Knowledge of a range of therapeutic interventions to the relevant client group, including group work IT skills excel, word, and e-mail Excellent report writing skills Excellent oral and written Communication skills Ability to work within and contribute to development of operational organisational policies Ability to work flexibly to meet competing demands Awareness and acceptance of the rights and needs of service users Full UK driving licence and access to vehicle for work purposes

Desirable

Understanding of national policy governing the delivery of adults and older peoples services A working knowledge of critical health and safety legislation Ability to negotiate and influence, and collaborate across professions. Motivational skills Clinical leadership and people management skills Understanding of governance and the implications for services, including audit, risk etc Skills in analysing and interpreting data and information Knowledge of broad human resources procedures

Qualifications Essential

Diploma/degree in Speech & Language Therapy HCPC registration Registered Member of the Royal College of Speech and Language Therapists Post graduate dysphagia qualification or equivalent to level of autonomous practice

Desirable

Evidence of a range of specialised, validated postgraduate courses. Relevant CPD portfolio

Experience Essential

Experience of managing own caseload autonomously Post-Graduate Clinical experience in community environments
