About this role
Job summary
The post holder will provide specialist learning disability liaison nursing support to adults with learning disabilities and/or autism accessing services at Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust (Doncaster Royal Infirmary, Bassetlaw Hospital, Mexborough Montagu Hospital) to ensure facilitation of open and easy access to the various departments in the acute services.
Main duties of the job
Manage a defined caseload of adults with learning disabilities and/or autism requiring specialist liaison input within acute hospital setting. Identify patients requiring reasonable adjustments and support DBTH to ensure relevant personalised care plans. Support admission planning, inpatient care and discharge processes Act as an advocate for people with learning disabilities accessing healthcare services. Provide specialist advice and support regarding: - Communication needs

- Consent to treatment
- Hospital passports
- Health inequalities
Participate in multidisciplinary meetings and case reviews.
Facilitate effective communication between acute staff, carers, community teams and other agencies.
Contribute to safeguarding processes in accordance with Trust policies.

Qualifications Essential
Registered RNLD Current NMC registration Degree level qualification or equivalent experience. Evidence of continuing professional development.
Desirable
Teaching qualification. Practice Assessor qualification.
Experience Essential
Experience working with adults with learning disabilities. Experience of multidisciplinary working. Experience of risk assessment and care planning. Experience supporting individuals with complex needs Experience working within safeguarding procedures. Experience delivering training and presentations.
Desirable
Experience working across acute and community services Experience in liaison nursing. Experience undertaking audit or quality improvement work.
Knowledge Essential
