We are looking to recruit an energetic, motivated and proactive person to support the Discharge Planning team and wards in ensuring smooth, timely patient discharges.This role is paramount in supporting patient flow throughout the trust, and maximizing bed capacity. The key focus for this role will include reviewing patient referrals for discharge support, ensuring consistency and response from relevant organisations in a timely manner.

You will support the wards regarding the initiatives used to support effective patient flow e.g. Red 2 Green SAFER and Criteria to Reside. You will role model excellent behaviours and ensure discharge planning is carried out to a consistent high standard.

This role is pan site and will involve weekend work.

Main duties of the job

To co-lead Discharge Planning initiatives, which promote a change-orientated organisational culture.

To ensure all local policies and discharge information reflect the Trust's Clinical Governance Strategy, ECIST and Department of Health guidance on discharge planning.

To alongside the team to ensure that patients with complex discharge needs are appropriately referred in line with Trust and National targets.

To ensure that trust staff are informed / able to access relevant discharge planning information to promote safe, timely and appropriate patient discharge from the acute setting.

To lead on the implementation of the Red2Green and SAFER initiatives support effective patient flow.

To contribute to 7 day service delivery in accordance with Care Act (2014).

An understanding of Local Discharge Planning Processes, Red2 Green/SAFER initiatives, Criteria to reside, SHOP principles Experience of MDT working both internal and external to the Trust. Can demonstrate the clinical decision making process. Awareness of discharge planning and patient flow initiatives to improve the patient's journey
