Come to the Lake District - the post is based at Penrith Community Hospital and is part of Eden ICC. This allows you to travel across the beautiful Eden Valley.
You will need to be experienced in managing patients with a variety of medical conditions and will be able to use your assessment and clinical reasoning skills to the full, supported by a friendly, multi-disciplinary team.
Our aim is to keep people in their own homes and out of hospital whenever possible, so ideally you will also have some community experience and be able to prioritise your caseload.
The Eden unit at Penrith Community Hospital is a 23-bedded hospital. The patients are predominantly elderly with the added challenge of multi-pathology, which will require you to adapt your approach according to each individual patients' need.
Main duties of the job
The majority of the role will be community based, requiring the post holder to provide rehabilitation to patients in their own homes around the Eden Valley. This may also include residential/ nursing homes or as outpatients. Having access to your own vehicle is therefore key.
You will be expected to cover triage, D2A's and urgent response visits.
Discharge to Assess (D2As) - require the ability to complete a functional assessment and the confidence to understand appropriate risk.
Urgent referrals (Rapid Responses) - may require staff to reschedule their caseload. Good organisational, time-management and prioritisation skills are essential.
Flexibility will be required as the post holder will provide support to the Eden unit as required. This ward-based work involves collaboration with the MDT to facilitate the patients' return back home. You will act as a role model promoting a rehab philosophy.
Good communication skills (verbal and written) and IT skills are essential for this post. You will need to communicate effectively with patients, their families and other health and social care staff.
You will need to triage new referrals on the phone, be able to write clear care plans, and use online diary and notes systems.
Experience of supervision and delegation is important, due to close working with our assistants and any students we support.
