Part time

We are looking for a Practice Nurse with experience in all basic Practice Nurse tasks to become part of the current nursing team. Skills and interest in diabetes would be particularly welcome. We are a friendly, motivated and mutually supportive nursing team within our 6 partner, training practice. You will be part of an expanding nursing team that includes 3 other Practice Nurses, 2 HCAs, ANP, SNA and a phlebotomist.

Hours

2 or 2.5 days per week. 20 - 25 hours.

Likely work pattern Mondays and Tuesdays.

Cover to 19.15 on Monday evenings and some early mornings and Saturday mornings to support enhanced access contractual practice opening hours.

Main duties of the job

Our ideal candidate will have previous experience in General Practice. You would be pivotal in supporting the Practice with its targets, chronic disease management and maintaining our standards of care for our practice population

The role involves delivery of all practice nurse services, working as part of the practice multidisciplinary team, having responsibility for a number of clinical areas including chronic disease management, health promotion and prevention, as well as supporting the management team in reviewing of clinical policy and procedure.

Skills and interest in diabetes/respiratory.

We use EMIS/Optum clinical system, Arden clinical templates, AccuRx, Docman and MedLink.

We are a 12,500 patient cohesive, welcoming and well-respected training practice of 6 partners, 6 salaried GPs, GP registrar, strong nursing team, broad pharmacy team, shared PCN staff : MSK practitioner, social prescriber and care coordinator. We are well supported by our management, admin, patient services & data teams. The practice provides a nurturing, developmental environment for all staff, recognising this as a priority to enable us to provide best care to our patients.

We are members of a small, 3 practice PCN.

Registered Nurse (NMC) Primary care experience

Experience Essential

All basic practice nurse tasks and duties. Experience of working in Primary Care/General Practice Experienced using EMIS and Ardens templates Support Infection Prevention Control Long term condition management (skills/interest in diabetes/respiratory) Support public health and screening programmes Promote health management and support work practice work on health inequalities and improving access. Work within local and national protocols Support delivery of enhanced services and public health campaigns (eg vaccination programmes) Provide travel advice services Provide wound care Contribute to practice targets (QOF etc) Support the practice team dealing with clinical emergencies Please see full specification attached.
