We are looking for a skilled, enthusiastic, and experienced Clinical Pharmacist to join the Walsall Division of Modality Partnership on a full-time basis (37.5 hours). In this role, you will be instrumental in delivering high-quality, patient-centred pharmaceutical care, optimising medicines safely and effectively, and driving innovation in Primary Care.

Main duties of the job
This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.
Key responsibilities include:
Clinically assessing, diagnosing, and prescribing (where qualified) Optimising medicines and improving prescribing safety Collaborating with a multidisciplinary team to enhance patient outcomes Supporting medication reviews and structured medicines optimisation Contributing to service development and innovative working practices Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities

Qualifications Essential
Masters degree in pharmacy (MPharm) or equivalent recognised pharmacy qualification. Registered as a Pharmacist with the General Pharmaceutical Council (GPhC), with no restrictions on practice. Independent Prescriber qualification, or willingness to work towards obtaining the qualification if not already held (subject to service requirements). Evidence of continuing professional development (CPD) relevant to clinical pharmacy practice.
Desirable
Postgraduate clinical pharmacy qualification (e.g. Clinical Diploma or equivalent). Membership or Fellowship of the Royal Pharmaceutical Society (RPS). Primary Care Pharmacy Education Pathway (PCPEP) qualification or evidence of working towards completion. Qualification or training in long-term condition management, medicines optimisation or prescribing.
Knowledge Essential
Strong understanding of medicines optimisation Excellent mathematical and scientific knowledge Knowledge of prescription drug legislation and regulations Understanding of long-term condition management Awareness of clinical governance in Primary Care Knowledge of local and national healthcare priorities
