About this role

Job summary

Due our longstanding practice pharmacist moving on to new adventures Hillside Practice is looking to recruit an experienced, highly-motivated clinical pharmacist to carry on the safe and cost-effective prescribing culture that has been embedded in the practice over the last decade.

We are a friendly & supportive multi-disciplinary team who appreciate the guidance of an innovative clinical pharmacist to lead us through the diverse requirements of medicines management in general practice whilst ensuring adherence to good practice in medicines optimisation, safe prescribing and improving patient care.

Main duties of the job

The successful candidate will provide expert, independent advice on medicines to GPs and other healthcare professionals, helping to support safe, effective, and cost-conscious prescribing. The role involves finding, understanding, and assessing medical research and other technical information to encourage the evidence-based and cost-effective use of medicines.

Provide advice to optimise patients' medication in order to maximise their treatment outcomes and minimise medicines-related waste, side-effects, interactions and waste.

Job description Job responsibilities

Medicines Optimisation

Promote safe, cost-effective, evidence-based prescribing. Support implementation of local and national prescribing guidance. Monitor prescribing data and identify opportunities for quality improvement. Lead on repeat prescribing processes and medication monitoring systems. Respond to MHRA alerts, drug safety updates, and national guidance. Assist in maintaining and reviewing practice formularies.Clinical Practice

Conduct annual medication reviews. Manage and optimise medicines for patients with long-term conditions. Support patients with complex medication needs. Prescribe safely within individual scope of practice as an Independent Prescriber. Identify, manage, and reduce risks associated with medicines. Signpost patients to the most appropriate healthcare professional as required.Patient Safety and Quality Improvement

Identify patients at risk from inappropriate prescribing. Participate in clinical audits and quality improvement projects. Support achievement of Quality and Outcomes Framework (QOF) and locally commissioned targets. Contribute to medicines-related significant event reviews and learning.Multidisciplinary Working

Work closely with GPs, nurses, healthcare assistants, community pharmacists, care homes, and secondary care teams. Attend multidisciplinary meetings where appropriate. Provide specialist medicines advice to the wider practice team & deliver medicines-related education to practice staff. Support the development of prescribing knowledge across the wider clinical team.Leadership and Service Development

Contribute to the development and implementation of pharmacy-led services. Support the Practice in delivering strategic objectives relating to medicines optimisation. Analyse and interpret prescribing and medicines management data. Promote innovation and continuous improvement in service delivery.Professional Responsibilities

Adhere to GPhC Standards for Pharmacy Professionals. Work within professional competencies and scope of practice. Maintain patient confidentiality and comply with GDPR requirements.
