About this role

Job summary

This is an exciting opportunity for you to join our dynamic, innovative and friendly team dedicated to improving our Inflammatory Bowel Disease (IBD) services. In this key post you will be working as part of an established multidisciplinary team and be responsible for providing a high quality pharmacy service with IBD.

The successful candidate will be expected to work Monday - Friday and participate in weekend (Saturday and Sundays) and Bank Holiday rotas.

Main duties of the job

As an enthusiastic, patient-centred Pharmacist, you will combine expert clinical knowledge with exemplary professional practice to enhance patient care and support the continued development of our Inflammatory Bowel Disease (IBD) service.

Working at the forefront of the IBD multidisciplinary team (MDT), you will play a key role in implementing, delivering, and continuously improving medicines optimisation and management for patients with IBD. Your responsibilities will include medicines reconciliation, promoting and undertaking medication reviews, and supporting the discharge process to facilitate high-quality patient outcomes. Through these activities, you will help reduce prescribing errors, optimise treatment, and ensure patient safety.

The Pharmacy Department offers comprehensive pharmaceutical services across site including a well-respected clinical pharmacy service supported by a dedicated TDM pharmacist, specialist clinical pharmacists and a well-equipped Medicines Information centre integrated into the Regional Medicines Information Centre. A large licensed 'specials' manufacturing unit complements these services.

Posts have been developed to ensure leadership roles are key with a high emphasis on delivering teaching and training to a wide range of healthcare professionals as well as managing teams. You will be able to develop senior roles and responsibilities and we encourage individuals to take on any new responsibilities to meet their development needs.

Person Specification

Qualifications Essential

Master in Pharmacy Degree Evidence of Continuing Professional Development Registered with the General Pharmaceutical Council Certificate in Pharmacy Practice or equivalent

Desirable

Diploma in Pharmacy Practice or equivalent diploma RPS membership Independent prescribing qualification

Experience Essential

Some experience in specialist field of practice Experience in providing / supporting a pharmaceutical service to wards and departments Previous experience in delivering training and education

Desirable

Experience of working within a range of clinical specialities Change management, audit and therapeutic review, critical appraisal experience

Skills Essential

Clinical pharmacy skills- demonstrates good clinical reasoning and judgment, manages difficult and ambiguous problems IT literate, including word processing, email and use of internet to search for evidence based medicine, knowledge of spreadsheet and database packages Ability to priorities a complex workload and to work accurately under pressure Enthusiasm /positive attitude, professional image and good team worker/ player
