About this role

Job summary

An exciting opportunity has arisen for a motivated and compassionate Band 5 Community Mental Health Practitioner to join Chorley Community Mental Health Team.

We are a friendly, supportive and forward-thinking multidisciplinary team committed to delivering high-quality, person-centred care. We work collaboratively with service users, families, carers and partner organisations to help people manage their mental health needs, build on their strengths and progress towards meaningful recovery.

Chorley CMHT values professional development, innovation and teamwork. You will receive regular clinical and managerial supervision and will be supported to develop your knowledge, confidence and community mental health practice.

Main duties of the job

As a Community Mental Health Practitioner, you will:

Undertake comprehensive, person-centred assessments.Develop, implement and review collaborative care and safety plans.Manage a defined caseload of people experiencing a range of complex mental health needs.Complete effective risk assessments, formulations and safety-management planning.Deliver evidence-based interventions that promote recovery, independence and wellbeing.Work closely with service users, families and carers.Monitor physical health and prescribed treatments, including psychotropic medication.Contribute to multidisciplinary meetings, clinical reviews and service-development initiatives.Work collaboratively with primary care, social care, inpatient services, urgent-care teams and voluntary, community, faith and social enterprise organisations.Maintain clear, timely and accurate clinical records.About you

Person Specification

Education/qualifications Essential

Professionally Registered Mental Health Nurse.

Knowledge Essential

Knowledge of mental illness and impact upon quality of life Pharmaceutical knowledge of medications: particularly psychotropics, Clozapine and Lithium Risk formulation Demonstrable interest in continuing developing knowledge Evidence of continuing professional development

Experience Essential

Post qualification experience within mental health services. With some of these to have been within acute mental health services. Experience of working with a full range of mental illness Experience of providing supervision. Demonstrable experience of providing teaching/training Audit Quality / Project development

Personal Essential

Skills in the use of methods of assessment, formulation / interventions, and evaluation Well-developed skills in the ability to communicate effectively, orally and in writing, complex and or clinically sensitive information to service user, their families, and other professional colleagues both within and outside the NHS. Well-developed skills in providing clinical supervision and consultation to other professional and non-professional groups. Ability to teach and train others, using a variety of multi- media materials suitable for presentations within public, professional, academic settings Ability to work sensitively and supportively with staff who are retraining and/or changing their way of working Leadership skills

Other Essential

The resilience necessary to overcome obstacles and resistance. Sensitivity towards organisational and political complexities. Good communicator. Evidence of being a team player.
