Main duties of the job
As a Senior Case Manager, you'll play a key leadership role within one of our community hub teams, helping people with complex needs achieve positive, lasting change.
You will:
Lead, supervise and support members of your Hub team.
Manage a caseload of individuals with complex needs.
Deliver high-quality case management, psychosocial interventions and harm reduction support.
Work closely with our clinical team to support prescribing interventions.
Build strong partnerships with local agencies to ensure people receive holistic, person-centred care that supports long-term recovery.
Our community hubs provide accessible, flexible services designed around the needs of the people we support.

Person Specification
Experience Essential
Microsoft Office including outlook, word and excel Supervisory experience Experience of working within substance misuse services Experience of a multi-disciplinary team working Experience of key working and psychological/psychosocial interventions Experience and understanding of case notes and report writing
Qualifications Essential
A relevant degree such as Psychology, Sociology, Social Work, OR Diploma in Adult Care (or equivalent) Level 3 Tackling Substance Misuse ILM Level 3 Leadership and Management (or equivalent)
