Job summary

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly QualifiedNurses.

We are seeking a dynamic and enthusiastic nurse to join our friendly team in East Kent. The Children's Therapies service provides Speech and Language Therapy, Occupational Therapy, Physiotherapy and Care Co-ordination services toCYP aged 0-19.

Care co-ordination is a service for babies and children (up to the age of 12) who present with an identified developing condition or disability which is impacting on, or has the potential to significantly impact upon their long-term developmentand/or function.

The post is suited to a nurse with experience of working with children and families in the community and who works well in a multidisciplinary team. We currently offer a hybrid work pattern for staff with a combination of face to face andvirtual working. Please see below for the main duties of the job.

Main duties of the job

o Provide assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care oridentified projects.oSupport in the supervision and development of staff.oActively work as a member of the children's therapies team to provide high quality care to service users.oAct as lead and support for care co-ordinators, managing the care co-ordination pathwaysWork with key colleagues (ie education and health) in order to support multi agency models of service delivery.The successful applicant will join our friendly therapies team. We pride ourselves on the support and induction offered tonew staff including; regular meetings with a line manager, access to supervision; in house and external training. We offerregular clinical governance days where all localities come together to share knowledge, skills and CPD experiences. As aband 6 you will be supported by Band 7 clinical leads from both within locality and across Kent.

Person Specification

Qualifications & Experience Essential

You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing You'll need to have experience of managing complex caseloads and supporting/ mentoring others You will need to have experience working with children and their families. You will be a registered health care professional with the Nursing and Midwifery Council. You will have at least one year's experience of working as a Nurse or midwife.

Knowledge & Skills Essential

You'll need have sound communication skills in order to understand and communicate both verbally and in writing You will need to have a customer focussed approach which is essential to; Provide patients, clients and their families with the best possible experience You will need to be able to be able to use IT systems and databases. Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks You will have knowledge of up-to-date children's health and well -being by using up- to- date evidence-based research and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker. About You Here's what you need to thrive in this role

Personal Attributes Essential
