Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
Join our Urgent Care Services team and play a vital role in helping people stay safe, independent, and well in their own homes. Our service provides short-term health, therapy and care support to individuals who may otherwise require hospital admission, while also supporting timely and effective hospital discharges back into the community.

Main duties of the job
As a valued member of our multidisciplinary team, you'll deliver delegated aspects of patient care under the guidance and supervision of registered nurses, therapists and healthcare practitioners. You'll support the assessment, implementation and review of care plans by monitoring and reporting patient progress, helping to ensure individuals receive the right care at the right time.
Your varied role will include undertaking clinical activities that you have been trained and assessed as competent to perform, such as recording vital signs, glucose monitoring, routine urinalysis, wound care, falls prevention support, equipment provision and assisting patients with activities of daily living. You'll help people build confidence and independence, delivering both individual and group interventions where appropriate. Working within established protocols and care pathways, you'll contribute to safe, effective and compassionate care, supporting patients to achieve the best possible outcomes while remaining within their home and community whenever possible.

Person Specification
Qualifications, training and experience Essential
You will have either: A Qualifications and Credit Framework (QCF) Level 3. This could be an NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare. Or you will be able to demonstrate the equivalent level of experience In addition you will also have Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C/ 4-9 or equivalent in English and Maths) You'll need to have recent experience of healthcare/ working as a health care assistant gained through education, work experience or previous employment Full UK/EU driving licence
Desirable
You may also have the Care Certificate, but if not, we will support you to achieve this within the first 3 months of starting.
Knowledge and Skills Essential
You will need to have a customer focussed approach Provide patients, clients and their families with the best possible experience You will need to be able to be able to use IT systems and databases You will understand where to signpost clients and families to alternative services to support behaviour change and improve health You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker
