About this role

• Understands and carries out pharmaceutical care in line with Vision and mission, Core Values, Goals and objective that suit the scope of the Pharmacy services and the hospital. • Understands the service organization structure. • Attend meetings held by the services in order to discuss issues and matters pertaining to the operation of the service. • Check and sign poison purchase orders prepared by the pharmacy purchasing assistant. • Perform, assists and supervises all activities in the pharmacy including dispensing, sales and supply/ dispensing of medications. • Contribute to in-service training for healthcare workers and provide counseling on the use and care of prescription drugs to patients. • Assess the educational needs of all the staffs and provide necessary training. • Gives examination to pharmacy staffs and to test and upgrade their skills. • Ensure that Dangerous Drugs and Psychotropic Drugs are handled, stored and issued according to laws and regulations and hospital policies. • Issue Psychotropic Drugs to nursing units. • Responsible in recording and filing of Psychotropic & Dangerous Drugs and to hold the keys of the cupboard. • Ensure prescription received are screened and interpreted correctly before dispensing. • Perform medication appropriateness review and validate any medication reconciliation when necessary • Check and endorse inpatient orders, outpatient prescription and TTA prescription (prescription intervention). • Assist pharmacist in charge in the evaluation of Staff as required. • Handle inquiries concerning drug contents, dosages, regimen review errors, storage and disposal. • Monitor drug therapies for drug interactions, allergies and contraindications. • Monitor and control inventory to ensure that drugs are available when needed and wastage is minimized. • Ensure and advises on safe disposal of pharmaceutical products. • Participate in quarterly and as when needed stock takes. • Check the prepared roster, verifies staffs annual leave prior to approval from Pharmacist in Charge. • Involve in identifying lapses/challenges in the services and contributes in solving and decision making and to conduct Root Cause Analysis in case of Medication Error. • Report and document adverse drug reactions, medication errors. • Involves in drug inventory procedures, inventory management, purchasing, receiving, drug supplies and medication storage of the services. • Keep work area tidy and clean. • Be precise and accurate while dispensing and in case of any error occurred to immediately report to Pharmacist in Charge. • Uphold best practices by using 7R rules - Right drug, right patient, right frequency, right duration, right route, right information and right documentation.
