Job summary
We are looking for a motivated and organised Band 6 Midwife to join our maternity Quality and Safety team, supporting the delivery of patient safety, governance, and perinatal mortality review processes across York and Scarborough Teaching Hospitals NHS Foundation Trust.
This is an excellent opportunity for a registered midwife with an interest in governance, learning from incidents, quality improvement, and improving outcomes for women, birthing people, babies, and families. Working under the direction of the Governance, Quality and Patient Safety Lead, you will support incident monitoring, PMRT processes, The ATAIN programme, audit activity, data collation, reporting, and the dissemination of learning across the multidisciplinary team.
The post holder will playan important rolein supportingtimely,accurate, and compassionate review processes, helping toidentifythemes,monitoractions, and contribute to improvements in maternity care. You will work closely with clinical, governance, bereavement, and multidisciplinary colleagues,maintaininghigh standardsof professionalism, confidentiality, and communicationat all times.
Main duties of the job
We are seeking someone who is detail-focused, compassionate, confident with data and documentation, and able to communicate sensitively in complex situations. You will be supported to develop your knowledge of governance, PMRT and ATAIN, audit, quality improvement, and national maternity safety requirements while contributing to a positive culture of learning and improvement.
Main duties will include:
Supporting PMRT coordination, data entry, case review preparation, and reporting requirements.
Assistingwith incident monitoring, escalation, Duty of Candour processes, and governance documentation.
Supporting audit, data validation, action tracking, and identification of themes and learning.
Contributing to quality improvement activity and dissemination of learning across maternity services.
Working collaboratively with clinical and governance colleagues to support safer, high-quality care.
Applicants must hold current NMC registration as a Registered Midwife and have experience working within maternity services. Experience of governance, audit, PMRT, incident review, or quality improvement processes would be an advantage, although support and development will be provided.

Qualifications Essential
Registered Midwife with current NMC registration. Evidence of continuing professional development relevant to clinical practice, governance, quality, or patient safety.
Experience & Knowledge Essential
Experience of working as a registered midwife within the NHS Understanding of clinical governance processes and patient safety systems Awareness of national maternity and patient safety guidance (e.g. PMRT, MBRRACE, NHS England) Knowledge of incident reporting processes Understanding of audit and quality improvement principles
Desirable
Exposure to audit, PMRT, or mortality review processes Understanding of the requirements of the Maternity Incentive Scheme Year 8 Safety Action C Awareness of CQC and regulatory standards Experience of working with incident reporting systems or governance databases
Skills & Abilities Essential
Strong organisational skills with the ability to prioritise workload, meet deadlines, and maintain accurate records. Good IT skills, including Microsoft Office and confidence using digital systems.
