An exciting opportunity has arisen for a full time Band 4 Pulmonary Rehabilitation (PR) Care Navigator. This is a new post working across Barking & Dagenham, Havering and Redbridge (BHR). The Care Navigator will be responsible for initial contact and triage of adults with a Respiratory diagnosis referred to BHR PR services, ensuring patients who are referred are appropriate for PR. Where patients are not suitable for PR, the care navigator will provide advice and guidance around self-management or signpost/refer to appropriate services. The role will also involve supporting PR across BHR as required.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

High Cost Area Supplement - Outer London This post also attracts payment for High Cost Area Supplement of 15%of the basic salary (minimum payment of £4,870 and a maximum payment of £6,1371 - per annum, pro rata for part time) .

Person Specification

Qualifications Essential

NVQ level 3 or equivalent standard of literacy and numeracy

Experience Essential

Minimum of 1 year UK experience in healthcare setting at a Band 3 or above Experience of working with and advising people with health problems, disabilities and older people

Desirable

Experience of working in a Respiratory team Experience working in a community team Experience developing person centred plans with clients/patients. Experience of working autonomously
