Are you an experienced Respiratory Nurse looking for a rewarding opportunity to make a real difference in community care?
Central London Community Healthcare NHS Trust is seeking a passionate and motivated Band 7 Respiratory Nurse Specialist to join our friendly and supportive Barnet Respiratory Service based at Oak Lane Clinic. This is a part-time, permanent opportunity to become part of a highly skilled multidisciplinary team delivering outstanding respiratory care across the London Borough of Barnet.
In this autonomous and varied role, you will provide specialist respiratory care for patients with COPD through our innovative Hospital at Home Service, nurse-led outpatient clinics and domiciliary home visits. You will play a key role in supporting patients to manage their long-term respiratory conditions, helping to reduce hospital admissions and improve quality of life.
Working closely with GPs, consultants, community teams and other healthcare professionals, you will contribute to delivering high-quality, person-centred care while supporting service development and continuous improvement.

Main duties of the job
To undertake comprehensive assessments of complex COPD respiratory patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning. To assess, develop and implement specialist care programmes, including home oxygen therapy, medication reviews, adhering to evidence based guidelines. To accurately undertake specialist clinical skills including performing arterial / capillary blood gases and spirometry. To interpret and analyse clinical and non-clinical tests to form an accurate assessment and suitability for the Hospital at Home scheme, co-ordinate appropriate services to ensure safe transfer of care to the community and develop comprehensive discharge plans. To work in a variety of clinical areas including patients' homes and out patient clinics To work as an autonomous practitioner accepting clinical responsibility for a specialised caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time To liaise with and give specialist advice to other members of the Multidisciplinary team (MDT) and Primary Care regarding the clinical management of patients with a COPD diagnosis. Have knowledge of COPD disease management pathways within the community, secondary and primary care setting

Person Specification
Education/Qualification Essential
Registered Nurse Current registration with NMC. Possess a Degree or equivalent experience Post basic qualification in Respiratory care or equivalent experience Qualification in Independent nurse prescribing or willing to undertake training
Experience Essential
Experience of working with respiratory patients Experience of working within a community setting. Demonstrates ability to develop new initiatives Demonstrates ability to managing change
Skills & Knowledge Essential
A working knowledge of clinical governance and evidence based practice Ability to demonstrate specialist clinical expertise Sound working knowledge of guidelines in the field of respiratory care Knowledge awareness of recent NHS policies/DoH agend Ability to demonstrate an understanding of the holistic needs of the client group Computer skills i.e. Word, data- bases email, internet

Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
