Main duties of the job

Key Responsibilities

Provide comprehensive primary care services to patients of all ages

Diagnose and manage acute and chronic medical conditions

Promote preventative healthcare and patient education

Maintain accurate clinical records and work collaboratively with the multidisciplinary team

Participate in clinical governance and continuous professional development

We are a cohesive and supportive partnership of 4 GPs with an established and experienced team serving a semi-rural population of 7,500 patients. We believe that we have the best practice team in the area! Most of our team have worked here many years and as a result we have a stable and committed workforce. We use SystmOne as it is used widely across the NHS which helps to support better communication and coordinated patient care.

Main Duties and Responsibilities:

Clinical responsibilities

The post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.

Providing counselling and health education.

Managing your patient caseload and referring to other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreed standards.

Collecting data for audit purposes.

Compiling and issuing computer-generated acute and repeat prescriptions.

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.

The post-holder will be required to visit patients during surgery time if clinically indicated.

Qualifications Essential

Full registration with the General Medical Council

Desirable

Excellent communication and clinical decision-making skills Commitment to delivering patient centred care
