About this role

Job summary

St Helena Hospice is seeking a Rehabilitation Assistant.

This is for 30 hours per week, working Mon, Tues, Weds, Friday 8am to 4pm

We are looking for a candidate who has an interest in developing their career in palliative rehab. This is a rewarding role, and you must be flexible and reactive to the needs of the service which can vary from day to day.

The role covers community and inpatient work, so a car driver is essential to the role.

Main duties of the job

To assist Occupational Therapists (OT) and Physiotherapists (PT) with day to day management of the community rehab waiting list and support of inpatients whilst maintaining high professional standards for patients.

To carry out administrative and housekeeping duties as delegated within the team.

Job description Job responsibilities

To understand OT and PT philosophy and process.

To understand the basic equipment that we supply.

To have some knowledge of palliative care conditions and the impact that this has on everyday function. To apply this knowledge to patients cases.

To have understand interdisciplinary teamwork and the respective roles within the group of staff.

Be able to communicate clearly and effectively both face to face and by telephone.

To attend the daily Hospice or Hospice in the Home MDT as directed by OT/PT

To liaise with the Hospice in the Home MDT members if they require support and plan visits for OT/PT as required.

To recognize and report unwell patients.

To gather information, screen and prioritise referrals for both OT and PT.

To assist with OT/ PT initial assessment, plans and evaluations.

To assess and provide basic mobility aids to patients, report to PT as appropriate and arrange for loan or replacement of equipment with statutory services.

To check and maintain PT equipment as required.

To assess and recommend basic daily living equipment reporting to OT as appropriate.

To implement therapeutic interventions such as exercise, stretching, energy conservation, pacing and relaxation, reporting to OT/PT as necessary.

To work alongside the MDT members to reinforce rehabilitation, independence and mobility.

To follow up community referrals and make follow up telephone calls for both OT and PT.

To carry out administrative tasks every day for the team members, to ensure the smooth running of the OT/PT workload such as ordering and chasing up equipment, processing referrals.

To be responsible for maintaining accurate patient care records in accordance with St Helena policy and procedures.

To be aware of and use safe manual handling procedures, as applied to patients, community equipment, wheelchairs, mobility aids and furniture.

To ensure bank stock is registered and re-ordered as needed.

In line with St Helena policy and procedures, review and reflect on own practice and performance through regular participation in clinical supervision and staff development reviews.

To keep a record of your training and developmental activities.

To contribute to the training of students within St Helena.

To participate in the collection of data/statistics and audit programme as required

Ensure you follow all policies and procedures in relation to prevention and infection control.

Person Specification

Experience Essential

Experience of working within a healthcare setting. Experience of working in a multidisciplinary team

Desirable

Experience of working as a rehab assistant. Palliative care experience

Communication Essential

Effective non-verbal, verbal and written communication skills. Good listening skills Interact and communicate effectively with other members of the MDT and external professionals.

Qualities Essential

Team player Mature outlook Able to keep calm and work under pressure Empathic Able to use own initiative Approachable, diplomatic and assertive.

Qualifications Essential

Full driving licence with Business Insurance Numeracy and Literacy skills i.e. GCSE Grade c or equivalent. Support Workers Access Course/NVQ Level III or equivalent.

Desirable

Microsoft Outlook/Office and Excel knowledge.

Skills and Knowledge Essential

Awareness of the concept of clinical and psycho-social care Excellent organisational skills. Computer and administration skills at basic level e.g. inputting of data into patient database & filing. Standard keyboard skills

Desirable

Ability to participate in group work Awareness of the holistic needs of patients with life- limiting illness
