Cognitive Care Navigator

Make a meaningful difference in the lives of older adults.

The Opportunity

As a Cognitive Care Navigator, you will support individuals experiencing memory loss, cognitive changes, dementia, and related conditions, along with their care partners and families. You will help them understand their options, access appropriate resources, and feel supported before, during, and after diagnosis.

You will provide cognitive care navigation, coordinate memory-screening initiatives, support caregivers, and contribute to community education and outreach. Working with Adult Day Services, Clinical Intake, Care Management, and other interdisciplinary teams, you will help promote early identification and coordinated support for individuals and families navigating cognitive change.

Compensation

$60,000 to $65,000.

Employment status

Full-time, salaried, exempt.

Schedule

Monday to Friday, 8:30 am to 5:00 pm.

Zoller/Kaplan Families Center for Cognitive Services

What You'll Do

• Cognitive Screening & Early Intervention

• Provides consultations, education, support, and referrals to individuals and care partners experiencing cognitive concerns.

• Coordinates the Alzheimer's Foundation of America (AFA) National Memory Screening Program and related screening initiatives.

• Maintains screening documentation, outcomes tracking, and program reporting.

• Supports cognitive health awareness, outreach, and early intervention efforts throughout the community.

• Cognitive Care Navigation

• Provides individualized cognitive care consultations for individuals experiencing memory concerns and cognitive changes and their care partners.

• Educates and supports individuals and families in understanding diagnoses, disease progression, care options, and available resources.

• Connects individuals and care partners with appropriate CJE programs, community services, and support.

• Assists with care planning, transitions, safety concerns, and other cognitive care needs.

• Offers ongoing guidance during periods of adjustment and decision-making.

• Family & Caregiver Support

• Provides short-term supportive counseling, emotional support, and problem-solving assistance to individuals and caregivers.

• Facilitates caregiver education, support, and learning opportunities and programs.

• Develops and maintains caregiver resources and educational materials.

• Community Outreach & Program Development

• Conducts presentations, workshops, and educational programs on cognitive health, dementia, caregiving, and related topics.

• Supports outreach efforts that increase awareness of CJE's cognitive services and available community resources.

• Assists with program development, implementation, evaluation, and growth initiatives within the Center for Cognitive Services.

• Supports grant activities, data collection, outcomes tracking, and reporting requirements.

• Interdisciplinary Collaboration

• Collaborates with Adult Day Services, Care Management, Clinical Intake, and other CJE departments to ensure coordinated support for individuals and care partners.

• Serves as an internal resource on dementia, cognitive impairment, caregiver support, and community resources.

• Maintains accurate documentation in accordance with agency standards, regulatory requirements, and HIPAA guidelines.

What You'll Bring

Required qualifications

• A master's degree in social work (MSW) is required.

• A minimum of 3 years of experience with older adults, caregivers, or individuals with dementia or related conditions is required.

• Experience in counseling, care coordination, navigation, caregiver support, assessments, and resource referral is required.

• Knowledge of ADRD, mild neurocognitive disorder, caregiver needs, and community resources is required.

Preferred qualifications

• Having or being eligible for an Illinois Licensed Clinical Social Worker (LCSW) licensure is preferred.

• Experience with outreach, public speaking, and program development preferred.
