About the role We're looking for an Assessment and Engagement Worker to join our Reconnected to Health team at HMP Holme House. This is a rewarding opportunity to support people affected by drug and alcohol use as they begin and progress through their recovery journey.
No two days are the same. You'll work closely with individuals from their first engagement with the service, carrying out comprehensive assessments, developing tailored recovery plans and helping them access the support they need to achieve their goals. Working alongside healthcare professionals, prison staff and partner organisations, you'll play a key role in improving health outcomes and reducing the impact of substance use within the prison environment.
This role offers the opportunity to make a genuine difference while working within a supportive multidisciplinary team.
What you'll be doing As an Assessment and Engagement Worker, you will:
• Complete comprehensive assessments and develop person-centred recovery plans. • Manage your own caseload, providing ongoing support throughout an individual's recovery journey. • Deliver evidence-based interventions on both a one-to-one basis and through group sessions. • Work collaboratively with colleagues and partner agencies to ensure seamless care. • Promote harm reduction, health and wellbeing, and recovery-focused approaches. • Assess and manage risk, safeguarding vulnerable adults and children where appropriate. • Maintain accurate records and contribute to achieving positive service outcomes.
About you We're looking for someone who is compassionate, resilient and motivated to support people facing complex challenges.
You'll ideally have:
• An NVQ Level 3 in Health & Social Care (or equivalent), or be willing to work towards one. • Experience delivering assessments and developing recovery or risk management plans. • Experience supporting individuals with multiple or complex needs. • Confidence delivering both one-to-one and group-based interventions. • An understanding of substance misuse and recovery approaches. • Strong communication, organisational and record-keeping skills. • A commitment to safeguarding, equality and delivering person-centred support.
