Main duties of the job
Key Responsibilities,
Deliver high-quality, evidence-based nursing care
Undertake diabetes reviews, monitoring, and patient education
Support management of long-term conditions (e.g. asthma, COPD, hypertension)
Provide routine nursing services including:
Immunisations and vaccinations
Wound care and dressings
Cervical cytology (if trained)
Health checks and screening
Promote health and wellbeing through lifestyle advice
Maintain accurate and contemporaneous clinical records
Work collaboratively with GPs and the wider multidisciplinary team

Main Duties of the Role
The Practice Nurse will:
Deliver nursing care to patients across a range of clinical areas Support the management of patients with long-term conditions, with a focus on diabetes Promote health and wellbeing through preventative care and education Work both independently and as part of the wider practice team Job Responsibilities
Clinical Care
Provide high-quality, evidence-based nursing care in line with NICE guidelines Deliver routine nursing services including: Wound care and dressings Suture and staple removal Cervical cytology Immunisations (routine, childhood, and travel) ECGs and spirometry Undertake new patient health checks and routine screening Chronic Disease Management
Support the management of patients with long-term conditions, including: Diabetes (essential) Asthma and COPD Hypertension Carry out reviews, monitoring, and patient education Support patients to self-manage their conditions Health Promotion & Prevention
Deliver opportunistic health promotion and lifestyle advice Support national and local health campaigns (e.g. vaccinations, NHS health checks) Promote patient wellbeing and disease prevention Infection Prevention & Control
Work in accordance with infection prevention and control policies Support IPC audits and compliance activities as required Medicines Management
Administer medications in accordance with PGDs and scope of practice Provide advice to patients on medication use, side effects, and compliance Patient Management & Administration
Request and process pathology tests as appropriate Maintain accurate, clear, and contemporaneous clinical records Refer patients to GPs or other healthcare professionals when appropriate Team Working
Work collaboratively with GPs, nursing colleagues, and the wider MDT Support colleagues and contribute to a positive team environment Participate in practice meetings and training Person Specification
Qualifications
Essential:
Registered Nurse (NMC) Diabetes qualification or relevant training/experience Experience in chronic disease management Immunisation and vaccination training
Desirable:
Cervical cytology training Experience

Experience of working in primary care Experience managing long-term conditions, particularly diabetes Experience working autonomously within scope of practice Desirable:
Experience with audit or quality improvement Experience with elderly or housebound patients Clinical Skills & Knowledge
Essential:
