Position Description POSITION TITLE: Expanded School Mental Health (ESMH) Grant QUALIFICATIONS: 1. Hold BSW. Current MSW student or recent MSW graduate preferred. 2. Must Subscribe to the NASW Code of Ethics. 3. Ability to function professionally within a multidisciplinary team that includes parents, guardians, caregivers, foster parents, kinship, etc. 4. Ability to maintain confidentiality. 5. Must have good organization and time-management skills. REPORTS TO: Principal and the Morgan County Lead Social Worker ESSENTIAL JOB FUNCTIONS: Planning/Devleopment: 1. Facilitate the monthly Mental Health Team Meetings in order to complete partner surveys, coordinate Tier 1 prevention progrmas. Establish student screening schedule with key school personnel. Select appropriate assessment tools for Tier 3 students. 2. Collaborate with school personnel and community agencies to provide services. Implementation of Programs: 1. Complete Strengths & Difficulties Questionnaire (SDQ) for students beginning Tier 3 services and follow up in 6 months or whene discharged from Tier 3. 2. Provide Brief Interviews and Brief treatment to all Tier 2 and Tier 3 students who are identified by assessment tools and rescreen students as appropriate. 3. Provide Support Groups to identified Tier 2 students. 4. Provide home visits to reach students who are absent from school due to suspension. 5. Review the Statement of Work (SOW) and perform additional job duties and responsibilities as arequired by the grant guidelines/SOW. Training/Consultation: 1. Licensed Independent Clinical Social Worker will provide on-site clinical supervision. 2. Funding for training is incorporated into the grant. 3. DHHR/BHHF and Marshall University have included changes to the SOW for the upcoming grant cfycle which may include summer training for Child and Adolescent Needs and Strengths, Children's Mobile Crisis Response, and Wraparound Services. Data Management & Report Submission: 1. Complete monthly ESMH reporting. 2. Pre/post reporting on Evidence based interventions. 3. Data Collection for Tier 2&3 interventions. 4. Data collection for assessments administered.
