Job summary
Over the last six years, our Clinical Pharmacy team have become a fundamental part of Mereside Medical Group Practices highly valued and appreciate by out GPs, Practice teams, and patients thanks to a track record of making a genuine difference to patient outcomes and GP workloads.
The teamcomprisesfour Clinical Pharmacists and three Pharmacy Technicians, working across three GP Practices - Cathedral Medical Centre in Ely, Haddenham Surgery, andStaploeMedical Centre in Soham.
Each Clinical Pharmacist is offered clinical supervision by a GP, and managerial supervision is offered by the Lead Pharmacist.

Mereside Medical is a flourishing group of three East Cambridgeshire practices in Ely, Haddenham and Soham, together serving c.45,000 patients. Your Responsibilities
Patient Care & Clinical Responsibilities
Undertake clinical medication reviews and Structured Medication Reviews, including for care home residents and patients with long-term conditions. Deliver patient-facing consultations (face-to-face, telephone, or digital),providingmedicines advice and managing referrals and acute queries, and assess minor conditions whereappropriate. Offer safety-netting, lifestyle advice, andappropriate signposting. Work collaboratively within multidisciplinary teams to optimise patient care.
Medicines Optimisation & Prescribing
Lead medicines optimisation to ensure safe, effective, and cost-efficient prescribing. Manage repeat prescribing and undertake medicines reconciliation to ensure safe transitions of care. Identifyand manage prescribing risks and support reduction in avoidable medicines-related hospital admissions.
Clinical Governance, Quality & Safety
Lead and support clinical audits, quality improvement, and service development. Use prescribing data to improve outcomes, manage high-risk patient cohorts, and implement local/national guidance. Support CQC compliance, incident reporting, and delivery of PCN DES, QOF, and public health priorities.
Leadership, Education & Services Development
Provide clinical leadership, including support for Pharmacy Technicians and Practice staff. Support development and improvement of prescribing systems and services.
Information & Professional Responsibilities
Maintain accurate clinical records and use digital platforms effectively. Maintain GPhC registration, CPD, and up-to-date clinical knowledge. Comply with governance requirements, including safeguarding, confidentiality, health and safety, and other mandatory training.
You should have or be:
Currentregistrationwith the General Pharmaceutical Council Completion of an undergraduate degree in Pharmacy Hold or be willing to work towards anIndependentPrescribing Qualification Minimumof 2 years as aPharmacist Experience and awareness of common acute andlong-termconditions that are likely to be seen in general practice Excellent communication skills Good understanding of primary care prescribing, medicines optimisation, clinical governance, and professional standards Able toidentifyand resolve risk management for long term conditions Demonstrate and understand conformity of relevant standards of practice Satisfactory DBS check and employment references

Experience Essential
Current registration with the General Pharmaceutical Council Completion of an undergraduate degree in Pharmacy Hold or be willing to work towards an Independent Prescribing Qualification Minimum of 2 years as a Pharmacist Experience and awareness of common acute and long-term conditions that are likely to be seen in general practice Excellent communication skills Good understanding of primary care prescribing, medicines optimisation, clinical governance, and professional standards Able to identify and resolve risk management for long term conditions Demonstrate and understand conformity of relevant standards of practice Satisfactory DBS check and employment references
