That person helps bring calm, organisation and good judgement. As Senior Care Coordinator you'll play a central role in keeping our service running safely and efficiently.

You'll work closely with the Registered Manager, coordinate day to day operations, support carers, communicate with professionals and help ensure clients receive outstanding care. This isn't simply a rota role. It's an operational coordination leadership role.

We're now looking for someone who can work alongside the Care Coordinator to ensure rota coordination is proactive, organised and responsive, helping the branch stay ahead of day-to-day operational pressures. This is the next step in management from the Care Coordinator role.

Job description Job responsibilities

The Senior Care Coordinator plays a key role in the safe, effective and organised delivery of homecare services across the branch.

The post holder will lead day-to-day service coordination, ensuring rotas, records, communication and operational processes support safe, reliable and person-centred care. Working closely with the Registered Manager, Deputy Manager, Care Coordinator and wider branch team, the Senior Care Coordinator will help keep the service responsive, well organised and focused on positive outcomes for clients.

This is an operational coordination leadership role. It requires strong organisation, sound judgement, clear communication and the ability to respond calmly and professionally when priorities change.

Key Responsibilities:

Service Coordination and Rota ManagementLead the day to day coordination of care delivery across the branch.

Work with the Care Coordinator to produce, maintain and improve weekly rotas.

Support rotas to be planned in advance wherever possible, while responding effectively to sickness, absence, holidays and urgent changes.

Help ensure rotas are safe, efficient and person centred for both clients and the care team.

Manage and improve client runs, including considering geography, continuity of care, staff availability and client needs.

Understand what makes a good rota for clients, carers and the wider service.

Respond to office alerts and operational issues promptly, escalating where appropriate.

Cover care calls during office hours when needed to support safe service delivery.

Arrange and allocate client introductions where required.

Support new care package onboarding and contribute to brokerage submissions with the Registered Manager.

Client, Family and Professional Communication

Act as a key point of contact for clients, families, carers and external professionals.

Liaise professionally with social workers, GPs, district nurses, occupational therapists, local authority representatives and other health and social care professionals.

Support clients by listening, recognising concerns and working with colleagues to find safe, appropriate solutions.

Identify and report changes in client wellbeing, health, presentation or risk.

Recognise potential safeguarding or mental capacity concerns and escalate appropriately.

Support client introductions, reviews and care coordination activity as required.

Maintain a compassionate, professional and responsive approach in all client and family communication.

Leadership and Team Support

Provide day to day guidance and support to the Care Coordinator and care team.

Help maintain a positive, organised and professional branch environment.

Support carers through clear communication, problem-solving and practical guidance.

Hold regular rota drop-ins or support sessions with the care team where required.

Mentor and support colleagues in the delivery of high-quality adult social care.

Provide constructive feedback and support reflective practice.

Help manage conflict or concerns through effective, professional communication.

Support team morale while maintaining appropriate professional boundaries and service standards.

Provide professional supervision where required and appropriate.

Quality, Safeguarding and Service Improvement

Support the delivery of safe, person-centred care in line with Avanti's values, policies and procedures.

Complete monthly visit note reviews and escalate concerns or themes as required.

Contribute to service improvement activity, pilots or projects where appropriate.

Identify recurring operational issues and work with branch leadership to improve ways of working.

Support good practice in safeguarding, mental capacity, risk management and professional accountability.

Use information from rotas, records, alerts, visit notes and feedback to help identify risks and improve service delivery.

Act flexibly, creatively and proactively when solving operational problems.

Systems, Records and Governance

Confidently use Avanti's digital systems, including care planning, rota and staff record systems.

Maintain accurate records for clients, carers, availability, rotas and service information.

Update People Planner records for staff and clients as required.

Input and maintain staff data, records and availability accurately.

Manage purchase orders and invoicing notes in collaboration with the Admin Executive and Derbyshire County Council.

Ensure all records and information are stored and managed in line with GDPR and company requirements.

Use Microsoft Office, email, online calendars and other digital systems effectively.

Be open to developing new digital knowledge and skills that improve performance and service coordination.

On-Call and Operational Flexibility

Participate in the management on call rota, including evenings and weekends, as part of the branch management team.

Respond to out of hours contact professionally and support the safe coverage of care calls.

Understand that homecare can be unpredictable and that the role requires flexibility, resilience and calm decision making.

Take responsibility for their own actions and demonstrate determination, impartiality and professionalism when managing difficult situations.

Person Specification

Qualifications Essential

The successful candidate will be able to demonstrate: Experience coordinating rotas, services, operational activity or people focused schedules. Experience working in a busy health, care, community, housing, service coordination or operational environment. Experience supporting colleagues, carers, team members or service users. Strong organisational skills and the ability to manage competing priorities. Sound professional judgement and the ability to decide when to act, when to escalate and when to seek support. Strong written and verbal communication skills. Confident use of digital systems to manage information accurately and support service coordination. Good attention to detail and accurate record keeping. A professional, compassionate and calm approach when speaking with clients, families, colleagues and professionals. Awareness of safeguarding principles and the importance of escalating concerns appropriately. Ability to work flexibly and remain calm when plans change. Full UK driving licence and access to your own vehicle. Ability to travel to clients homes when required.

Desirable

Previous experience in homecare, domiciliary care, supported living, healthcare, housing, community services or another regulated service. Experience using care planning, rota, workforce management or case management systems. Experience supervising, mentoring or supporting colleagues. Experience completing visit note reviews, care records, introductions, brokerage information or service coordination documentation. Health and Social Care qualification. Knowledge of safeguarding, mental capacity, person-centred care or care planning.
