Job summary

We are seeking an experienced and motivated Nurse Practitioner to provide autonomous clinical care to patients across a broad range of acute and chronic conditions; a respiratory background would be preferred. This is an exciting opportunity to work across both sites as part of a supportive and collaborative team.

Our two practices, who are based within the same building, on the same floor, and share the waiting area, serve a diverse patient population and have a strong focus on proactive, preventative care. Key Responsibilities

Deliver high-quality clinical care, including assessment, diagnosis, and treatment of patients Diagnose and manage acute and long-term conditions. Independently prescribe medications within your scope of practice. Order, interpret and act upon diagnostic tests and investigations. Support long-term condition management and proactive patient care, providing health advice, disease prevention and lifestyle advice. Refer patients to secondary care and specialist services when required. Maintain accurate and timely patient records. Work collaboratively with GPs, practice nurses, ARRS staff, and the wider MDT Contribute to service development and quality improvement initiatives

Job Summary

The Nurse Practitioner (NP) is an advanced practice registered nurse responsible for providing high-quality, patient-centred healthcare services. The NP assesses, diagnoses, treats, and manages acute and chronic health conditions, promotes health and wellbeing, and collaborates with multidisciplinary healthcare teams to deliver comprehensive care. The role includes conducting clinical examinations, ordering and interpreting diagnostic tests, prescribing medications within scope of practice, and educating patients on disease prevention and management.

Key Responsibilities

Clinical Care

Conduct comprehensive patient assessments, including medical histories and physical examinations. Diagnose and manage acute and chronic illnesses. Develop, implement, and evaluate treatment plans. Prescribe medications and therapies in accordance with professional guidelines and regulatory requirements. Order, interpret, and act upon laboratory and diagnostic test results. Provide urgent, routine, and preventive care services. Patient Education and Support

Educate patients and families regarding diagnoses, treatment options, and self-management strategies. Promote health maintenance, disease prevention, and healthy lifestyle choices. Support patients in making informed healthcare decisions.

Care Coordination

Collaborate with physicians, nurses, pharmacists, and other healthcare professionals. Coordinate referrals to specialists and community services as required. Participate in multidisciplinary team meetings and case reviews. Documentation and Compliance

Maintain accurate, timely, and confidential patient records. Ensure compliance with professional, legal, regulatory, and organisational standards. Adhere to safeguarding, infection prevention, and clinical governance requirements. Quality Improvement

Contribute to service development and quality improvement initiatives. Participate in audits, research activities, and evidence-based practice projects. Support the implementation of clinical policies and procedures. Qualifications Essential

Registered Nurse with current registration with relevant professional body. Qualified Nurse Practitioner with relevant prescribing qualification - or working towards. Strong clinical assessment and decision-making skills

Experience Essential

Strong clinical assessment and decision-making skills. Demonstrated experience in clinical assessment, diagnosis, and treatment planning. Competency in prescribing medications (where applicable). Strong knowledge of clinical guidelines and evidence-based practice. Ability to work autonomously and as part of a team. Excellent communication and patient engagement skills. A proactive and compassionate approach to patient care. Ability to work autonomously and as part of a team.

Desirable

Experience in primary care, urgent care, family practice, or specialty services. Experience managing chronic disease clinics. Special interest in Respiratory Diseases Lead on respiratory care, including COPD and asthma reviews, management, and optimisation in line with best practice Knowledge of UK primary care systems and pathways. Experience using SystmOne.
