Position Summary The CNA provides direct patient care under the supervision of a Registered Nurse (RN). This role focuses on supporting patients with daily living activities, promoting comfort and safety, and assisting the clinical team in delivering high-quality care.
Key Responsibilities
• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating. • Monitor and report changes in patient condition to nursing staff promptly • Assist with patient mobility, including transfers, repositioning, and ambulation • Provide emotional support and companionship to patients and families • Maintain a clean, safe, and comfortable patient environment, including changing linens and sanitizing equipment • Document care provided and update patient records accurately • Assist nurses and other healthcare professionals with procedures and care tasks
Qualifications & Requirements
• Active Certified Nursing Assistant (CNA) certification in the state of Texas. • High school diploma or equivalent • Current CPR and proof of auto insurance • Valid Driver’s License • Must pass drug test • Basic knowledge of patient care practices and medical terminology • Ability to follow care plans and instructions from nursing staff • Strong communication and interpersonal skills • Ability to perform physical tasks, including lifting up to 50 lbs. and standing for extended periods • Knowledge of Point Care charting system preferred but not required.

Work Environment CNAs work in a variety of healthcare settings such as hospitals, nursing homes, assisted living facilities, and hospice care.
Physical Requirements
• Frequent standing, walking, bending, and lifting • Ability to safely move or assist patients • Manual dexterity for patient care tasks • • JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
• At least 18 years of age. • Ability to read and follow written instructions and document care given. • Understands hospice philosophy, comfortable providing specialized care to the terminally ill. • Is self-directing with the ability to work with little direct supervision. Secure with issues of death/dying. • Provides a calm manner when in a patient’s home. • Has empathy for the needs of the ill, injured, frail and impaired. • Possess and maintains current CPR certification. • Demonstrates tact, patience, and good personal hygiene. • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order. • Good oral and written communications • Good organizational skills • Knowledge of Pointcare EMR charting system—preferred but not required.
