Connect Cares is a forward-thinking foster care agency dedicated to supporting youth aged *12 to 18* who face complex behavioral, emotional, and mental health challenges.
*Responsibilities*
* Provide *individualized, compassionate, and consistent support* to children and youth with developmental disabilities, behavioural crises, and other complex needs, ensuring their safety, dignity, and well-being. * Deliver services in *diverse environments* including live-in treatment programs, residential care homes, foster or family homes, schools, hospitals, and community-based settings. * *Accurately monitor, record, and maintain* all required documentation, including daily shift logs, incident reports, medication charts, and case notes, in compliance with agency and legal standards. * Assist with *meal planning and preparation*, promoting balanced nutrition and encouraging healthy eating habits. * Ensure that all living environments are *clean, safe, welcoming, and conducive to growth*. * Administer prescribed medications responsibly, following agency protocols, and complete *Medication Logs* with precision. * Provide safe and reliable *transportation* for clients to and from medical appointments, school, recreational programs, and community events. * Support clients in developing and maintaining *daily living skills*, including personal hygiene, self-care, and household responsibilities. * Foster *trusting, professional, and respectful relationships* with clients, families, caregivers, and multidisciplinary team members. * Demonstrate flexibility and commitment by *travelling between multiple work sites* to meet the needs of clients. * Respond to behavioural or emotional crises with *calm, skilled interventions* that promote safety and de-escalation. * Actively participate in *team meetings, training, and continuous learning opportunities* to enhance skills and service quality.
*Qualifications & Requirements*
* *Education:* Diploma in Child and Youth Work, Personal Support Work, Social Work, Developmental Services Work, or a related human services field. * *Certifications:* * Valid *CPR and First Aid* certification (required) * Certification in *Understanding and Managing Aggressive Behaviour (UMAB)* (required) * *Screening & Clearances:* * Current *Vulnerable Sector Check* or *Broad Record Check* (completed within the last 6 months) (required) * *Child Welfare Check* (required) * *Licensing & Transportation:* * Valid *G2 or G Driver?s License* (required) * Access to a *personal, reliable vehicle* (required) * Flexibility to travel to various work locations as needed * *Work Schedule:* Ability to work *12-hour shifts* (required)
