Community Health Representative Summary:Reporting to the Health Services Manager, and working with the Community Health Nurse, the Community Health Representative promotes the health and well-being of Zagim Anishinabk members through health education, prevention, outreach, and community support. Duties and Responsibilities: Plan and deliver community health education, awareness, and prevention programs.Coordinate and administer the Childrens Oral Health Initiative.Conduct home and hospital visits for seniors, individuals with disabilities, and chronic care clients.Connect community members with health programs and provide follow-up support.Assist the Community Health Nurse with monitoring, and follow up education, and regarding communicable diseases.Support public health education at Goose Lake School and Zagim Daycare/Headstart.Recommend health programming improvements and share community feedback.Prepare monthly activity and statistical reports.Assist the health team in delivering programs aligned with the Multi-Year Workplan and funding agreements.Support maternal/child health and home care services as required.Perform other related duties as assigned. Qualifications: Post-secondary education in a related field or an equivalent combination of education and experience.Minimum two years of relevant experience.Computer proficiency, including data entry.Knowledge of First Nations health issues, culture, traditions, and community structures.Experience coordinating public health education and awareness initiatives.Strong communication, organizational, and interpersonal skills.Ability to work independently, follow direction, and maintain confidentiality.Reliable, dependable, and willing to work flexible hours when required.Valid Class 5 driver's licence, reliable vehicle, and eligibility to operate Band vehicles.Satisfactory Criminal Record Check with Vulnerable Sector Screening prior to employment.
