About this role

Job summary

To provide high-quality, patient-centred general medical services to the practice population, delivering safe, effective and evidence-based care within a multidisciplinary team environment. The postholder will undertake a mixture of routine clinical consultations, triage and associated clinical administration, while contributing to teaching, quality improvement and the ongoing development of innovative models of care.

Main duties of the job

Provide high-quality, patient-centred primary care to a diverse patient population.

Undertake a mixture of routine clinical sessions, triage responsibilities and associated clinical administration.

Work collaboratively within a highly skilled multidisciplinary team including GPs, Advanced Nurse Practitioners, Physician Associates, non-prescribing clinicians and other healthcare professionals.

Contribute to the safe and effective management of same-day demand through our team-based triage model.

Participate in clinical governance, quality improvement and service development initiatives.

Support the training and development of GP Registrars and other members of the multidisciplinary team where appropriate.

Promote evidence-based practice and contribute to maintaining high clinical standards across the organisation.

Engage with practice and community-based initiatives designed to improve patient health and wellbeing.

Participate in home visiting where clinically appropriate, although the majority of visits are undertaken by the PCN Home Visiting Team and GP home visits are infrequently required.

We are a progressive GP training practice serving approximately 15,500 patients across our three sites in Shoeburyness, Thorpe Bay and Westcliff-on-Sea (All in the city of Southend-on-Sea). Our multidisciplinary team includes a GPs, Advanced Nurse Practitioners, Practice Nurses, Prescribing Nurses, Physician Associate, a Clinical Pharmacy team and a highly skilled administrative and care navigation team.

Job description Job responsibilities

Clinical Duties

Provide face-to-face, telephone, and online consultations for acute, chronic, and preventative care. Undertake home visits where clinically appropriate. Make professional, autonomous decisions in relation to patient assessment, diagnosis, and treatment. Manage long-term conditions in line with local and national guidelines. Provide timely review of laboratory/test results and communicate findings. Prescribe medications safely and appropriately in accordance with national and local prescribing policies. Work collaboratively within the MDT, including ANPs, Nurses, Pharmacists, Social Prescribers, and Mental Health Workers. Support management of patients with complex or multi-morbidity needs. When triaging to ensure same-day requests are actioned in a safe and timely manner.

Support urgent appointments, telephone triage, and clinical queries for reception/administrative teams. Participate in the practice rota . Administrative Duties

Maintain accurate and contemporaneous medical records. Contribute to QOF, IIF, Enhanced Services, and other practice performance targets. Assist with clinical audits, significant event reviews, and quality improvement projects. Provide support and clinical leadership for practice policies, protocols, and governance activities.

Person Specification

Skills & Competencies Essential

Excellent clinical assessment and diagnostic skills. Strong communication and interpersonal abilities. Ability to work well under pressure and prioritise workload. Commitment to holistic, patient-centred care. Ability to work effectively within a multidisciplinary team.

Qualifications Essential

Full registration with the GMC and inclusion on the GP Register. Evidence of ongoing CPD and Revalidation. Membership of a recognised medical defence organisation. Eligibility to work in the UK. Experience in UK General Practice preferred. Enhanced DBS check.
