About this role

Job summary

We are seeking an experienced First Contact Physiotherapist (FCP) to provide autonomous assessment, diagnosis and management of musculoskeletal conditions in primary care. You will work as the first point of contact for patients without the need for prior GP assessment, in line with the NHS First Contact Practitioner Roadmap.

Main duties of the job

Provide autonomous first contact assessment, diagnosis and management of MSK presentations.

Identify red flags and serious pathology and arrange urgent referral where appropriate.

Request and interpret investigations within local pathways.

Provide self-management advice, exercise prescription and onward referrals.

Contribute to education, audit and quality improvement.

Essential

HCPC registered Physiotherapist; significant MSK experience; excellent clinical reasoning; FCP Roadmap verified or working towards verification; commitment to high-quality patient care.

About us

Morris House is situated in Tottenham in a large, modern and airy purpose-built Medical Centre, which we share with district nurses, community physiotherapists, podiatrists, psychologists, tissue-viability nurses and a sexual health clinic. Our list size is about 15,000 patients. It is a diverse area and a cultural melting-pot.

Our practice has a very strong teaching ethos. We train GP registrars, medical students, physicians associates and foundation year 2 doctors among others. We have close links with Kings Medical School and the St Georges Physician Associate teaching program. We provide training to GP trainees from the Enfield and Haringey VTS and Foundation Year 2 doctors from the North Middlesex Hospital rotation program.

We perform the full range of clinical roles that you would expect in primary care. In addition, we offer high level minor surgery and specialist tongue tie release services. We have Diabetic, Learning Disability, Spirometry clinics. We offer a range of face to face and telephone appointment.

We are a friendly and interactive practice. We support our staff with study leave allowance, study leave funding, regular meetings, morning coffee, teaching and social events / parties. We conduct regular satisfaction surveys.

Clinical ResponsibilitiesAssess undifferentiated MSK conditions using advanced clinical reasoning. Develop evidence-based management plans. Refer directly to secondary care, community services and imaging where appropriate. Maintain accurate EMIS records and coding. Work collaboratively with the multidisciplinary team.Leadership, Education and Service Development Support colleagues with MSK education. Participate in teaching and supervision. Lead and contribute to audit, QI and pathway development.Professional Responsibilities

Communicate highly sensitive condition related information to patients, relatives and carers. Initiate the process of diagnosis with patients suspected to have a chronic disease

Strategic Development of Primary Care Services

Contribute to the strategic development of primary care and public health services to meet patient and population needs within the practice. Work/liaise across multi-professional and agency boundaries. Work at the forefront of practice innovation. Determine how to deal with ambiguous or unique problems to develop and advance primary care services for the benefit of patients, carers and communities. Recognition and involvement in Morris House Group Practice as a teaching practice and help drive the further development of the clinical teams by offering expert physiotherapist advice and teaching Develop links within the Primary Care Network and other local and national bodies relevant to primary care physiotherapy initiatives.

Health Improvement

Initiate and carry out programmes of health screening and ensure other team members are aware of health agendas and screening programmes relevant to physiotherapy. Introduce innovation in practice to meet ongoing health need demands of the practice population. Share innovation and new learning with others. Support practitioners in profiling the practice population in order to initiate contribute and promote improved health and prevent disease in individuals and groups. Represent individuals and families interests when they are not able to do so themselves. Make referrals to statutory and voluntary agencies as appropriate.

Communication

Provide and receive highly complex, sensitive information. Use developed communication, negotiation and management skills Regularly communicate service-related information to Partners, Practice Manager, Salaried GPs, Nursing, AHPs, Administrative staff, patients, relatives and carers. Make operational judgements, manage conflicting views and reconcile inter and intra professional differences of opinion. Build and communicate therapeutic working relationships with a wide array of statutory and voluntary organisations for the benefit of patient care and facilitates good working relationships. Develop and deliver presentations to groups

Management & Leadership

Aim to develop a special interest (dependent on experience) within the practice Encourage and motivate others to be forward thinking in ideas that address the health needs of the practice that will be innovative whilst managing and developing new ways of working, Develop evidence-based practice, standards and promote best practice. Promote effective communication and relationships within the team. Actively participate in clinical and practice meetings. Develop own knowledge and practice to meet objectives/changes in service, through attendance on study days, self-directed learning and reflection in practice. Integrate theory into practice and establish formal links with educational institutions where appropriate. Operate high standards of probity. Implement and adhere to practice/local/national, clinical commissioning group protocols and pathways, propose developments and improvements to service delivery.Manage and Use Information

Make effective use of the clinical system by ensuring the consistent and accurate entering of data and use of Read Codes. Facilitate the use of up to date information technology in order to collate accurate and timely information as and when required by the practice. Ensure a satisfactory call and recall system of patients. Assist in the development of practice protocols for the surgery and clinical protocols for the clinical team. Identify, monitor and review deficiencies in procedures and suggest remedial action.

Person Specification

Experience Essential

Minimum 5 years' post-registration physiotherapy experience Minimum 3 years' specialist MSK physiotherapy experience Experience of autonomous assessment and management of complex MSK presentations Knowledge of NICE guidance and evidence-based MSK practice Experience of multidisciplinary working Understanding of the NHS, Clinical Governance and Information Governance
