As a trust bank Pre-Assessment nurse you will be able to choose a flexible working pattern to suit your work life balance as part of a committed multidisciplinary team.

This role is predominantly based at CHASE FARM HOSPITAL . However, there may be opportunities to cover other sites as well.

Main duties of the job

The post holder will offer Royal Free World Class Value based Care to service users, staff, colleagues, clients and patients alike.

The Pre-Assessment Nurse will have the below duties as part of wider responsibilities.

1.The post holder is responsible for the assessment, implementation and evaluation of care given to patients within the Preoperative Assessment service.

2.The postholder will work closely with other staff from all disciplines to ensure an efficient and effective service is provided to patients.

3.To provide clinical expertise and assist with organisational skills in the efficient running of the Unit.

4.To assist the senior sister with daily management of the unit.

5.To assist with rostering and allocations ensuring area is appropriately staffed demonstrating an awareness of the guidelines and escalation process for staffing shortages.

Job description Job responsibilities

The post holder's major responsibilities are as follows:-

CLINICAL PRACTICE

1.To demonstrate clinical expertise in the Pre Assessment Unit.

2.To be responsible for the assessment of patient care needs and the development, implementation and evaluation of plans of care, ensuring high standards of evidence based care using a defined model of care.

3.To have a good knowledge of all procedures carried out and to ensure relevant information is given to patients.

4.Assess individual care needs of all patients attending the unit to ensure suitability for day surgery/in-patient surgery.

5.To undertake all nursing procedures in accordance with Trust and POA policies and ensure that these procedures and policies are understood. POA policies and procedures must be research based and adhered to by all staff whilst being constantly monitored and reviewed.

6.To ensure communication with patients and significant others is established and maintained, ensuring empathy and reassurance regarding, assessment, care and treatment.

7.To promote patients health education.

8.To ensure accurate and comprehensive nursing records, in all formats, are maintained using appropriate documentation.

9.To support effective working relations with all members of the multi-disciplinary team to promote good communication and a multi-disciplinary approach to patient care.

10.Demonstrate knowledge of clinical practice, explore new ideas and introduce positive evidence-based change in line with local and national initiatives, e.g. National Service Frameworks, Essence of Care, NICE guidance and WHO guidelines.

11.To support the Sister / Charge Nurse and Matron in carrying out risk assessments according to Trust Clinical Governance/Clinical Risk Strategies.

12.To support the implementation of safe working practices as required by the Health & Safety at Work Act.

INFECTION CONTROL RESPONSIBILITIES

1.To take responsibility for own evidence based practice and implement and add to the body of knowledge that supports infection control and prevention within the department.

2.To support the Team Leader with audits that are key to improve service delivery.

3.To maintain a culture in which infection control is intrinsic to care delivery. To ensure that this is reflected in all staffs PDPs.

OPERATIONAL/MANAGEMENT

1.To be responsible for the day to day effective and efficient management in conjunction with the Sister / Charge Nurse.

2.To ensure that all staff are working within Trust guidelines and protocols.

3.To liaise with other members of the multidisciplinary team to improve patient information within the unit.

LEADERSHIP

1.To act as an appropriate and effective role model at all times.

2.To act as an effective mentor and preceptor to support all staff in promoting education and training.

PROFESSIONAL

1.The post holder is required to act in accordance with NMC Code of Professional Conduct with particular reference to limitations of competence and conduct.

2.To be responsible for ensuring an awareness of current developments in Nursing, the Health Service and Preoperative Assessment.

RESEARCH & DEVELOPMENT

1.To be aware of the research process and current trends.

2.To carry out audits in a timely manner and disseminate findings to appropriate personnel.
