Job summary
A rewarding opportunity has arisen for a Community Mental Health Nurse within the Intensive Life Skills Team based in Shropshire.
We are a specialist community team working within the area of complex emotional needs providing assessment, formulation and intervention primarily using a Structured Clinical Management (SCM) framework.Our focus is on treatment, recovery and enablement to increase emotional social and practical functioning, through a combination of group skill based learning and individual therapy.
The post is a band 5-6 development role and the appointed applicant will have the opportunity to access further development and training within this role.
Main duties of the job
As part of this role, you will be working in a specialist adult community mental health pathway, providing evidenced based interventions for an identified caseload of service users. You will be responsible for undertaking assessments in accordance with the clinical pathway, to establish the person's needs, goals, risks, and strengths, to inform and generate a formulation-based care plan, agreed with all involved. Responsibility for delivering evidenced based therapeutic interventions on a 1:1 and group basis, in accordance with an agreed care plan.
