Job summary
We are searching for a Diabetes Care Co-ordinator to join our Gloucester Inner City PCN Diabetes team. You will be supporting the Specialist Diabetes Nurse to identify patients with complex diabetes on the GP systems and to make sure they get the care they need.
You will be setting up clinics and booking patients in, sending appointment reminders and following up with patients who do not attend their appointments.
This position would be suitable for the right candidate that is willing to learn and is interested in working in health care services. Full training can be given on the systems if you have not worked for a GP surgery before.
You will be required to work across various locations in Gloucester. This will include our Inner City PCN Surgeries Gloucester Health Access, Pavilion and St James Family Doctors, Severnside Medical Practice and Kingsholm Surgery.
Working Hours:16 hours per week. Fixed term for 2 years.

Main duties of the job
To work with the Specialist Diabetes Nurse to use the GP systems to identify people with complex diabetes to ensure they get the care and support they need.
To provide administrative support for the Specialist Diabetes Nurse and the Diabetes Project.
To book appointments, send reminders and follow up with patients who do not attend.
Supporting MDT meetings including taking minutes.
To work with social prescribers and community providers to ensure people are signposted to the right support.
The job is primarily working with practices in the PCN and you will be required to travel independently between practices and occasionally G DOCs offices in Gloucester, and to attend meetings etc, hosted by other agencies throughout Gloucestershire.

Main duties of the role
Provide administrative support, working for the benefit of patients, providing and maintaining high standards of care for patients health needs.
The post holder will become an integral part of the practice multidisciplinary team, working alongside the specialist nurse, social prescribing link workers and other community providers to provide an all-encompassing approach to personalised care.
The role is intended to support the PCN team to develop services and achieve the contractual targets, undertaking a variety of projects and supporting digital transformation as required.
You may be required to travel independently to other Gloucester Inner City PCN practices and to attend meetings etc., hosted by other agencies throughout Gloucestershire.
Reports to (line manager): PCN Business Manager
Duties
Identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.
Book clinics, set up appointment reminders and follow up with patients who do not attend their appointments.
Work closely with GPs and practice team to manage patients to develop individual personalised care and support plans, ensuring appropriate support is made available to patients and carers, helping them to understand and manage their condition and ensure changing needs are addressed.
Review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing
Liaise and work with the ICB, practices, voluntary and other organisations to implement patient services
Assist the Business Manager in collating information for claims (e.g. ARRS, Care Homes, etc) and responding to queries
Monitor allocated mailboxes and respond appropriately to emails
Coordinate, organise and attend various MDT meetings
Assist the Lead education GP to organise training
Support the PCN team to effectively deliver key projects, including quality improvement projects. This includes the provision of information and analysis using information, data, knowledge and technology systems to inform and support practice initiatives and meet the needs of the local population.
Support the PCN Leadership team in the delivery of the PCN DES specifications and support Quality and Outcome Frameworks.
Assess data, monitor dashboards and liaise with the Digital and Transformation Leads, Practices, GPs and Nursing teams, with a view to achieving targets and obtaining maximum investment and impact funding
Take minutes of meetings as required.
Raise awareness of health promotion in practices, implementing, co-ordinating and supporting a variety of projects.
Helping to ensure that the practice remains safe and effective, including by the use of clinical audit and learning from significant events and complaints.
