Job summary
We are looking for an enthusiastic and compassionate ADHD Social Prescriber to join our innovative ADHD Primary Care Connect Service in Brighton & Hove.
This is an exciting opportunity to provide short-term, person-centred support to adults newly diagnosed with ADHD, helping them develop practical strategies, improve day-to-day functioning and access appropriate community support.
Working as part of a multidisciplinary team, you will provide tailored one-to-one support for up to six weeks, empowering individuals to build confidence, increase independence and make the most of local services and resources.
This is a fixed-term pilot, with the possibility of extension subject to future funding.
Main duties of the job
As the ADHD Social Prescriber, you will:
Provide structured one-to-one support for adults following an ADHD diagnosis.
Co-produce personalised support plans focusing on organisation, routines, emotional wellbeing, employment, education and social connection.
Use coaching and strengths-based approaches to build sustainable self-management skills.
Connect individuals with local community organisations, peer support, employment services and wellbeing resources.
Manage a caseload of clients receiving short-term interventions.
Maintain accurate clinical records and outcome data.
Work closely with GPs, ADHD clinicians, primary care teams and voluntary sector partners.
Identify safeguarding concerns and escalate appropriately.
Contribute to the ongoing development and evaluation of the ADHD Primary Care Connect Service.

The successful candidate will provide personalised, strengths-based support to adults recently diagnosed with ADHD, helping them adjust following diagnosis and develop practical coping strategies.
Responsibilities include:
Delivering one-to-one support for up to six weeks.
Supporting individuals to improve organisation, time management and daily routines.
Helping people understand their diagnosis and identify practical adjustments.
Signposting and connecting individuals to local community resources and voluntary organisations.
Supporting referrals into longer-term services where appropriate.
Managing an allocated caseload and maintaining accurate records.
Working collaboratively with healthcare professionals and community partners.
Contributing to service evaluation and ongoing development.
Following safeguarding procedures and recognising when clinical escalation is required.
Person Specification
Qualifications Essential
GCSE (or equivalent) in English and Maths. Educated to Level 3 (A-Level or equivalent) or relevant experience within health, social care or community services.
Desirable
Social Prescribing qualification (or equivalent).] Training in health coaching, behaviour change or brief interventions.
Personal Qualities Essential
Compassionate and empathetic. Motivated and proactive. Solution-focused. Uses initiative and sound judgement. High level of integrity. Able to work calmly under pressure.
Experience Essential
Experience supporting adults within health, social care, voluntary or community settings. Delivering person-centred support. Caseload management. Supporting individuals with emotional wellbeing needs.
Desirable
Experience as a Social Prescriber or Link Worker. Experience supporting adults with ADHD or other neurodivergent conditions. Experience working within primary care or multidisciplinary teams.
Knowledge and Skills Essential
Understanding of adult ADHD and its impact on daily life. Knowledge of social prescribing and community support. Excellent communication and interpersonal skills. Ability to build rapport quickly. Strong organisational and time management skills. Ability to maintain accurate records. Ability to motivate individuals towards positive change.
Desirable
Knowledge of local services across Brighton & Hove. Understanding of coaching or behaviour change techniques.
