We have an exciting opportunity for an enthusiastic individual with the skills, drive, and commitment to join our service as an Enhanced Care Specialist Practitioner/Team Lead and make a real difference to the people we care for.

This role combines specialist clinical practice with operational leadership of the Enhanced Care and Support Team, including participation in a 7-day rota (8am-8pm). The post holder ensures safe, effective, and consistent care for patients requiring enhanced observation and therapeutic support, including people with mental health needs, people living with dementia, people experiencing delirium, people with learning disabilities, autistic people, and people with eating disorders.

Working closely with matrons, multidisciplinary teams, and specialist leads, the post holder leads the standardisation of enhanced care processes in line with professional and statutory guidance. They undertake holistic assessments, support risk management and care planning, and coordinate resources to ensure safe staffing and high-quality care.

You will be providing expert clinical advice, leadership, and education, supporting a skilled and competent team. You'll also contribute to governance, audit, and service improvement, promoting a culture of learning and excellence while improving patient outcomes, experience, and safety.

Main duties of the job Provide strong clinical and operational leadership to the Enhanced Care and Support Team, acting as a visible role model and upholding Trust values. Lead and coordinate shifts, ensuring safe staffing and high standards of care for patients requiring enhanced observation and support. Work in partnership with ward teams and the multidisciplinary team to assess, plan, and review care, including managing risk and supporting complex decision-making. Deliver expert clinical practice, including holistic assessment, triage, care planning. Provide specialist advice on mental health, legal frameworks, and person-centred care to support safe and effective practice. Support and develop staff through supervision, coaching, training, and performance management, acting as line manager for Enhanced Care Practitioners. Promote a positive learning culture and ensure staff are appropriately trained, competent, and supported in their roles. Drive quality improvement by contributing to audit, governance, policy development, and service development. Ensure effective communication with patients, families, carers, teams, and partner agencies to support coordinated care. Monitor standards of care, respond to incidents or concerns, and take action to maintain safety and quality. Encourage evidence-based practice, contributing to research, audit, and the dissemination of learning to improve patient outcomes

Person Specification

Qualifications & Training Essential

Registered Mental Health Nurse Current NMC registration Training in restrictive intervention, de escalation, and conflict resolution, including awareness of the risks of physical restraint and associated medical considerations (if not already completed, this must be undertaken within 3 months of commencing the role

Desirable

Leadership and management training Relevant post registration qualification or postgraduate diploma Quality Improvement (QI) methodology training Nurse prescribing qualification

Knowledge & Experience Essential

Evidence of managerial, leadership, and organisational skills
