An exciting opportunity has arisen to join the NationalHospital forNeurology&Neurosurgery as a MotorNeuroneDisease(MND)CareCentreCoordinator(Band7).
This is a fixed term post for 12months covering maternity leave, supported by a MotorNeuroneDiseaseAssociation(MNDA)grant.
Working within a nationally recognised MND Care Centre, you will contribute to the coordinated, proactive and multidisciplinary care model recommended in NICENG42. The role focuses on collaboration, working closely with the MNDClinicalNurseSpecialist, AdvancedNursePractitioner, ConsultantNurse, Neurologists and the wider MDT to ensure seamless, person centred support from diagnosis through to ongoing management.
You must have an understanding of the needs of patients with a life limiting illness; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with a willingness to learn about Motor Neurone Disease (MND)

Main duties of the job
The Band7MNDCareCentreCoordinator will hold an appropriate professional qualification (RNorAHP) and work collaboratively within the MNDteam and wider multidisciplinary network to deliver coordinated, proactive and person centred care in line with NICENG42 guidance.The postholder will:o Practice autonomously while working in partnership with the MNDClinicalNurseSpecialist, AdvancedNursePractitioner, ConsultantNurse, Neurologists and community based MDTs.o Coordinate care for people living withMND by attending borough specific MDT meetings, facilitating referrals and maintaining accurate records on the MNDregister.o Contribute to the development and review of protocols, pathways and organisational guidelines, ensuring compliance with NICENG42 and the NationalServiceFramework forLong TermConditions.o Act as a clinical resource for colleagues and students, promoting evidence based practice and supporting education within the specialist area.o Participate in audit, service evaluation and quality improvement initiatives to enhance patient experience and service delivery.o Continue to develop specialist clinical knowledge and apply research and critical analysis to practice.o Build strong clinical networks and maintain effective communication across acute, community and voluntary sectors.o Demonstrate commitment to equality, diversity and inclusion, and uphold the Trust's values of safety, kindness, teamwork and improving.
